2007—2018年浙江省海宁市结直肠癌发病与死亡趋势分析

摘　　要：[目的]分析海宁市自2007年开展结直肠癌早诊早治项目以来,居民结直肠癌发病率、死亡率及变化趋势。[方法]收集2007—2018年海宁市户籍人口学资料和结直肠癌发病、死亡数据,计算发病率、死亡率和年度变化百分比(APC)。[结果]2007—2018年海宁市结直肠癌新发病例共2555例,占全部恶性肿瘤发病的10.03%,居癌症发病谱第2位,粗发病率为31.74/10万,中标发病率16.89/10万,世标发病率为16.65/10万。其中男性新发病例数为1419例,女性为1136例,男性中标发病率为女性的1.24倍。结直肠癌年龄别发病率在40岁以前处于较低水平,40岁以后随年龄增长呈现快速上升,至80~84岁年龄组达到高峰(170.52/10万)。2007—2018年海宁市结直肠癌粗发病率和中标发病率均呈上升趋势,APC分别为4.7%(95%CI:2.8%~6.6%,P<0.001)、1.9%(95%CI:0.2%~3.6%,P=0.036),男性增速高于女性。2007—2018年海宁市结直肠癌死亡病例共1094例,占全部恶性肿瘤死亡的7.94%,居癌症死亡谱第4位,粗死亡率为13.59/10万,中标死亡率为6.65/10万,世标死亡率为6.48/10万。其中男性死亡病例数为608例,女性为486例,男性中标死亡率为女性的1.24倍。不同年龄别死亡率在40岁以前处于较低水平,40岁以后随年龄增长呈现快速上升,至80~84岁年龄组达到高峰(132.63/10万)。2007—2018年海宁市结直肠癌粗死亡率呈上升趋势(APC=2.2%,95%CI:0.2%~4.1%,P=0.032),其中男性呈上升趋势(APC=4.0%,95%CI:1.6%~6.4%,P=0.003),而女性呈下降趋势(APC=-0.1%,95%CI:-3.0%~3.0%,P=0.968);中标死亡率呈下降趋势(APC=-1.7%,95%CI:-4.1%~1.0%,P=0.168),其中女性中标死亡率下降趋势(APC=-3.5%,95%CI:-6.6%~-0.3%,P=0.034)比男性中标死亡率(APC=-0.2%,95%CI:-3.1%~2.7%,P=0.856)显著。[结论]海宁市结直肠癌中标发病率呈缓慢上升趋势,中标死亡率呈下降趋势,持续多年的早诊早治项目长期效应已开始体现,但仍需进一步加强结直肠癌的早诊早治和预防�[Purpose]To analyze the trends of incidence and mortality rates of colorectal cancer in Haining City from 2007 to 2018.[Methods]The demographic data of Haining registered residence and the incidence and death data of colorectal cancer from 2007 to 2018 were collected,and the incidence rates,mortality rates and annual percentage change(APC)were calculated.[Results]From 2007 to 2018,there were 2555 new cases of colorectal cancer reported in Haining City,accounting for 10.03%of all malignant tumors ranking the 2nd of all cancers.The crude incidence rate was 31.74/105,age-standardized incidence rate by Chinese standard population(ASIRC)was 16.89/105,and by world standard population(ASIRW)was 16.65/105.The number of new cases was 1419 in male and 1136 in female,and the ASIRC in male was 1.24 times higher of that in female.The age-specific incidence rate of colorectal cancer was at a low level before the age of 40,and showed a rapid increase after the age of 40,reaching a peak in the age group of 80~84 years old(170.52/105);the crude incidence rate and the ASIRC were both on the rise from 2007 to 2018,with APC of 4.7%(95%CI:2.8%~6.6%,P<0.001)and 1.9%(95%CI:0.2%~3.6%,P=0.036),respectively;and the APC of males was higher than that of females.There were 1094 deaths of colorectal cancer in Haining City from 2007 to 2018,accounting for 7.94%of all cancer deaths,and ranking the 4th.The crude mortality rate was 13.59/105,the age-standardized mortality rate by Chinese standard population(ASMRC)was 6.65/105,and by world standard population(ASMRW)was 6.48/105.The number of deaths was 608 in male and 486 in female,and the ASMRC in male was 1.24 times higher of that in female.The mortality rate was at a low level before 40 years of age,and showed a rapid increase after 40 years old,reaching a peak at the age group of 80~84 years old(132.63/105).The overall crude mortality rate of colorectal cancer in Haining City showed an increasing trend from 2007 to 2018(APC=2.2%,95%CI:0.2%~4.1%,P=0.032),there was an increasing trend for males(
