恩格菲辅助非肌层浸润性膀胱癌患者术后吡柔比星膀胱灌注化疗的效果观察

摘　　要：目的:探究恩格菲辅助非肌层浸润性膀胱癌患者术后吡柔比星膀胱灌注化疗的效果。方法:选取2020年4月—2022年1月我院收治的76例NMIBT患者,均接受经尿道膀胱肿瘤电切术,按照随机数字表法1∶1配对原则分为观察组(n=38)、对照组(n=38),其中对照组术后给予THP膀胱灌注,观察组术后给予恩格菲辅助THP膀胱灌注,比较两组膀胱功能、细胞因子[血管内皮生长因子(VEGF)、成纤维细胞生长因子(FGF)]、Ki-67高表达率、钙黏蛋白(CDH)13甲基化率、肿瘤标志物[糖类抗原125(CA125)、膀胱肿瘤抗原(BTA)、癌胚抗原(CEA)]及安全性。结果:治疗12个月后,观察组排尿次数、尿急、尿失禁评分低于对照组(P<0.05);观察组血清VEGF、FGF水平低于对照组(P<0.05);观察组Ki-67高表达率、CDH13甲基化率低于对照组(P<0.05);观察组CA125、BTA、CEA水平低于对照组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:恩格菲辅助THP膀胱灌注应用于NMIBC患者术后可有效调节细胞因子、肿瘤标志物水平,降低CDH13甲基化率、Ki-67高表达率,改善膀胱功能,具有一定安全性。Objective:To explore the efect of Engelf assisted pirarubicin bladder perfusion chemotherapy in patients with nonmuscle invasive bladder cancer.Methods:A total of 76 patients with NMIBT admitted to our hospital from April 2020 to January 2022 were selected.All patients underwent transurethral resection of bladder tumor.They were divided into observation group(n=38)and control group(n=38)according to the 1:1 matching principle of random number table method.The control group was given THP bladder perfusion after operation,and the observation group was given Engefei assisted THP bladder perfusion after operation.Bladder function,cytokines[vascular endothelial growth factor(VEGF),fbroblast growth factor(FGF)],high expression rate of Ki-67,methylation rate of cadherin(CDH)13,tumor markers[carbohydrate antigen 125(CA125),bladder tumor antigen(BTA),carcinoembryonic antigen(CEA)]and safety were compared between the two groups.Results:After 12 months of treatment,the scores of urination frequency,urgency and urinary incontinence in the observation group were lower than those in the control group(P<0.05).The levels of serum VEGF and FGF in the observation group were lower than those in the control group(P<0.05).The high expression rate of Ki-67 and the methylation rate of CDH13 in the observation group were lower than those in the control group(P<0.05).The levels of CA125,BTA and CEA in the observation group were lower than those in the control group(P<0.05).There was no signifcant diference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Engelf assisted THP intravesical instillation in patients with NMIBC can efectively regulate the levels of cytokines and tumor markers,reduce the methylation rate of CDH13 and the high expression rate of Ki-67,improve bladder function,and has certain safety.
