摘  要:目的目前的研究已经证实骨质疏松症与牙周炎之间存在一定关联,但是二者之间的因果关联尚不清楚。同时,血清25-羟基维生素D状态也可改变牙周炎的发生风险。因此,本研究进行了孟德尔随机化(Mendelian randomization,MR)分析探究骨密度(Bone mineral density,BMD)与牙周炎之间是否存在因果关联,以及是否由血清25-羟基维生素D介导。方法本研究采用骨密度、血清25-羟基维生素D以及牙周炎的全基因组关联研究(Genome wide association studies,GWAS)的汇总统计数据进行了两样本孟德尔随机化研究,以探究骨密度水平和牙周炎风险之间是否存在因果关联,以及是否由血清25-羟基维生素D介导。同时,本研究进行了敏感性分析,以确保结果的稳健性。结果在IVW固定效应模型下,遗传预测的骨密度与牙周炎风险之间存在因果关联并呈负相关(OR=0.921,95%CI:0.855-0.991,P=2.71e-02),最大似然法(OR=0.921,95%CI:0.856-0.991,P=2.84e-02)。对骨密度和血清25-羟基维生素D水平进行两样本孟德尔随机化研究,加权中位数结果显示,骨密度与血清25-羟基维生素D水平相关(OR=0.986,95%CI:0.973-0.999,P=3.44e-02),惩罚加权中位数分析(OR=0.983,95%CI:0.970-0.997,P=1.72e-02)。对血清25-羟基维生素D水平和牙周炎进行两样本孟德尔随机化研究,MR-Egger分析结果显示,血清25-羟基维生素D水平与牙周炎风险相关(OR=1.203,95%CI:1.012-1.430,P=3.95e-02)。结论MR分析研究显示,骨密度与牙周炎之间存在因果关联,并且可能由血清25-羟基维生素D介导。Objective Osteoporosis and Periodontitis have been shown to have a relationship in current studies.However,the causal effect remains unclear.At the same time,serum 25-hydroxyvitamin D status can also change the risk of periodontitis.Therefore,Mendelian randomization(MR)analysis was performed in this study to investigate whether there is a causal association between bone mineral density(BMD)and periodontitis,and whether it is mediated by serum 25-hydroxyvitamin D.Methods The summary statistics of genome-wide association studies on BMD,serum 25-hydroxyvitamin D and periodontitis were used in this study.This research performed a Mendelian randomization analysis on two samples to determine whether BMD level and periodontitis risk were causally related and whether it is mediated by serum 25-hydroxyvitamin D.The MR analysis was performed mainly with the inverse-variance weighted(IVW)method.A sensitivity analysis was conducted to ensure that the results were robust.Results Under the IVW fixed effects model,genetically predicted BMD was causally and negatively associated with periodontitis risk(OR=0.921,95%CI:0.855-0.991,P=2.7le-02),Maximum likelihood(OR=0.921,95%CI:0.856-0.991,P=2.84e-02).In the two-sample Mendelian randomization study of bone mineral density and serum 25-hydroxyvitamin D levels,weighted median results showed that bone mineral density was associated with serum 25-hydroxyvitamin D levels(OR=0.986,95%CI:0.973-0.999,P=3.44e-02),penalised weighted median analysis(OR=0.983,95%CI:0.970-0.997,P=1.72e-02).A two-sample Mendelian randomization study was conducted on serum 25-hydroxyvitamin D level and periodontitis,and the results of MR-Egger analysis showed that serum 225-hydroxyvitamin D level was associated with risk of periodontitis(0R=1.203,95%CI:1.012-1.430,P=3.95e-02).Conclusion MR Analysiss studies haveshownacausal association between1 bonemineral densityand periodontitis,which may be mediated by serum 25-hydroxyvitamin D.
