女性抑郁症患者应激感受和环状RNA在应激性生活事件与抑郁程度间的链式中介作用

摘　　要：目的:探索应激感受和潜在生物标志物环状RNA hsa_circ_0006862的表达在应激性生活事件和抑郁程度之间的作用路径。方法:2019年1月至2020年8月选取30名女性抑郁症患者和30名健康女性作为研究对象,采集外周血作为环状RNA的来源,进行实时荧光定量聚合酶链反应(quantitative real-time polymerase chain reaction,qRT-PCR)以验证hsa_circ_0006862的差异表达,并通过受试者工作特征(receiver operating characteristic,ROC)曲线评估其诊断价值。对30名女性抑郁症患者进行问卷调查,选用自编一般情况调查表、汉密尔顿抑郁量表、社会再适应评定量表、应激感受量表为评估工具。使用SPSS 23.0对数据进行分析,采用Spearman相关分析考察应激性生活事件、应激感受、hsa_circ_0006862表达量与抑郁程度之间的相关性。使用Bootstrap法进行中介效应检验,探究应激感受和hsa_circ_0006862表达量在应激性生活事件和抑郁程度之间的中介作用。结果:qRT-PCR结果表明抑郁组hsa_circ_0006862表达量高于对照组(Z=-6.402,P<0.001)。根据ROC曲线分析,hsa_circ_0006862的曲线下面积为0.762(95%CI=0.637~0.887,P<0.01)。相关分析表明,应激性生活事件与应激感受(r=0.54,P<0.01)、抑郁程度(r=0.81,P<0.01)呈正相关,与hsa_circ_0006862表达量呈负相关(r=-0.78,P<0.01);hsa_circ_0006862表达量与应激感受(r=-0.69,P<0.01)、抑郁程度(r=-0.98,P<0.01)呈负相关;应激感受与抑郁程度呈正相关(r=0.67,P<0.01)。中介效应分析发现,应激性生活事件对抑郁程度的直接效应显著,效应量为0.512(95%CI=0.271~0.753)。hsa_circ_0006862表达量在应激性生活事件和抑郁程度之间的中介效应显著,效应量为0.228(95%CI=0.076~0.365)。应激感受和hsa_circ_0006862表达量在应激性生活事件和抑郁程度之间的链式中介效应显著,效应量为0.100(95%CI=0.024~0.213)。结论:应激性生活事件会直接影响女性抑郁症患者的抑郁程度,也可以通过应�Objective To explore the pathways of stress perception and expression of potential biomarker circular RNA hsa_circ_0006862 in the relationship between stressful life events and depression level in female patients with depressive disorder.Methods Thirty female depressive patients and thirty healthy females were selected as the study subjects from January 2019 to August 2020,and peripheral blood was collected as the source of circular RNA.Differential expression of hsa_circ_0006862 was verified by quantitative real-time polymerase chain reaction(qRT-PCR).And its diagnostic value was assessed by receiver operating characteristic(ROC)curve.Thirty female depressive patients were investigated by self-administered general information questionnaires,Hamilton depression scale,social readjustment rating scale,and perceived stress scale.SPSS 23.0 software was used for data analysis.Spearman correlation analysis was used to investigate the correlation between stressful life events,stress perception,hsa_circ_0006862 expression and depression level.The mediating effect test was conducted using Bootstrap method to explore the mediating effect of stress perception and hsa_circ_0006862 expression between stressful life events and depression level.Results qRT-PCR result showed that the expression of hsa_circ_0006862 in depressive patients was higher than that of health controls(Z=-6.402,P<0.001).According to ROC curve analysis,the area under the curve of hsa_circ_0006862 was 0.762(95%CI=0.637-0.887,P<0.01).The correlation analysis showed that stressful life events was positively correlated with stress perception(r=0.54,P<0.01)and depression level(r=0.81,P<0.01),and negatively correlated with hsa_circ_0006862 expression(r=-0.78,P<0.01).hsa_circ_0006862 expression was negatively correlated with stress perception(r=-0.69,P<0.01)and depression level(r=-0.98,P<0.01).There was a positive correlation between stress perception and depression level(r=0.67,P<0.01).The mediating effect analysis showed that stressful life events had a signifi
