摘　　要：帕金森病(PD)是一种临床表现异质性很高的慢性、进展性、多神经系统受累疾病。随着病情逐渐进展运动并发症的出现患者神经功能进一步恶化,生活质量下降及寿命缩短。常规传统药物治疗难以满足患者良好控制症状,此时称为PD晚期(Advanced PD,APD)。如何早期识别APD并结合病情变化、及时制定患者最佳治疗方案,在调药基础上推荐PD设备辅助治疗,不仅可提高患者服药依从性和治疗满意度,还可提高患者生活质量及减少照料者负担,但目前仍缺乏识别APD的临床表现和诊断的金标准。为此,本文旨在从APD早期识别和治疗策略方面进行阐述。Parkinson's disease(PD)is a chronic and progressive nervous system disorder that exhibits high clinical heterogeneity.As the disease advances,patients experience further deterioration of their neurological function and a decrease in their quality of life.Advanced PD(APD)refers to a stage where conventional drug treatments are no longer effective in controlling the symptoms of PD.Early detection of APD and timely development of an optimal treatment plan,such as recommending PD device therapy in addition to drug adjustment,can not only enhance patient compliance and treatment satisfaction,but also improve the overall quality of life for patients and alleviate the burden on caregivers.However,there is currently no universally accepted standard for identifying the clinical manifestations and diagnosing APD.Therefore,the objective of this article is to discuss strategies for early identification and treatment of APD.
