2011—2022年盐城市盐都区居民食管癌死亡趋势分析

摘  要:目的了解2011—2022年盐城市盐都区居民食管癌死亡状况,分析居民食管癌死亡的趋势和特征,为制定癌症预防控制策略提供科学的依据。方法收集盐城市盐都区2011—2022年死因登记资料,计算食管癌死亡率、年龄别死亡率、标化死亡率(SMR)、去死因期望寿命(CELE)、去死因期望寿命增长年(PGLEs)、寿命损失率,用Joinpoint回归模型拟合食管癌死亡趋势。结果2011—2022年盐城市盐都区因食管癌死亡3965人,年均死亡率为46.30/10万,其中男性死亡率为57.31/10万,女性为34.34/10万,男性高于女性(P<0.05)。随着年龄的增长,死亡率呈上升趋势(χ^(2)_(趋势)=9022.08,P<0.05);≥55岁人群食管癌死亡率高于35~<55岁人群,差异有统计学意义(χ^(2)=9624.22,P<0.05)。2011年食管癌死亡率为40.13/10万,标化死亡率为32.83/10万,2022年食管癌死亡率为43.26/10万,标化死亡率为19.74/10万,2011—2022年盐都区食管癌标化死亡率呈下降趋势(AAPC=-6.95%,P<0.05);去死因期望寿命增长年下降了0.18岁(AAPC=-1.98%,P<0.05);寿命损失率下降了0.25%(AAPC=-2.60%,P<0.05)。结论盐城市盐都区居民食管癌死亡率12年来总体呈下降趋势但仍处于较高水平,≥55岁人群尤其是男性仍为防控重点人群,今后仍需加强健康教育,倡导居民改善膳食结构,继续以早诊早治为抓手,从而减少食管癌发生和死亡。Objective To understand the status of esophageal cancer deaths in Yandu district,Yancheng city from 2011 to 2022,analyze the trend and characteristics of esophageal cancer deaths,and provide scientific evidence for the formulation of cancer prevention and control strategies.Methods The death-cause registration data from 2011 to 2022 in Yandu district,Yancheng city were collected;esophageal cancer mortality,age-specific mortality and standardized mortality rate,cause eliminated life expectancy(CELE)and potential gains in life expectancy(PGLEs),and life loss rate were calculated;the trend of esophageal cancer deaths with Joinpoint regression model was fitted.Results From 2011 to 2022,3965 people died from esophageal cancer in Yandu district,Yancheng city,and the annual mortality rate was 46.30/106(5).The mortality rate was 57.31/106(5)in males,which was higher than that in females(34.34/106(5),P<0.05).The mortality rate increased significantly with increasing age(χ^(2)_(trend)=9022.08,P<0.05).The mortality rate of people≥55 years old was higher than that in people 35-<55 years old(χ^(2)=9624.22,P<0.05).In2011,the mortality rate of esophageal cancer was 40.13/106(5),and the standardized mortality rate was 32.83/106(5).In 2022,the mortality rate of esophageal cancer was 43.26/106(5),and the standardized mortality rate was 19.74/106(5).From 2011 to 2022,the standardized mortality rate showed a downward trend(AAPC=-6.95%,P<0.05);PGLEs decreased by 0.18 years(AAPC=-1.98%,P<0.05);the life loss rate decreased by 0.25%(AAPC=-2.60%,P<0.05).Conclusions The mortality rate of esophageal cancer in Yandu district,Yancheng demonstrated a declining trend during the past 12 years,but it is still at a high level;the people≥55 years old,especially men are still the focus of prevention and control;in the future,it still needs to strengthen health education,advocate residents to improve dietary structure,and continue to early diagnosis and early treatment for reducing the occurrence and death of esophageal cancer.
