急诊无X线透视治疗急性化脓性胆管炎的临床价值

摘　　要：[目的]探讨无X线透视急诊内镜下胆道支架置入术治疗急性化脓性胆管炎的安全性及临床价值。[方法]回顾性分析307例急性胆管炎患者的临床资料。根据手术方式,分为不透视组86例、透视组221例。比较2组患者的临床资料、并发症发生率及手术成功率。[结果]不透视组高血压、冠心病、服用抗凝药比例明显高于透视组,2组比较差异有统计学意义。前者手术操作时间远低于后者(P=0.000)。2组均无穿孔及死亡患者,并发症及手术成功率比较均差异无统计学意义。[结论]无X线透视下行内镜下胆道支架置入术可有效解除急诊重症高危患者的胆道梗阻症状,手术成功率高,且不增加术后并发症发生率,在急诊患者中安全可靠。[Objective]To explore the safety and clinical value of emergency endoscopic biliary stent placement in the treatment of acute suppurative cholangitis without X-ray fluoroscopy guide.[Methods]This was a retrospective analysis on 307 patients with acute suppurative cholangitis.According to the severity of the disease and the way of surgery,they were divided into two groups,of which 86 cases in the non-X-ray group and 221 cases in the X-ray group.There was no perforation and death in both groups.There was no statistical difference between the two groups in complication and surgical success rate.[Results]The operation time in the non-X-ray group was much lower than the X-ray group(P=0.000).The proportion of patients with hypertension,coronary heart disease,and anticoagulants taken in the non-X-ray fluoroscopy group was significantly higher than that in the X-ray fluoroscopy group,and there were statistical differences.[Conclusion]Endoscopic biliary stent placement without X-ray can effectively relieve the symptoms of biliary obstruction in emergency high-risk patients with high success rate and without increasing the incidence of postoperative complications.
