TCT联合宫颈组织活检对宫颈癌高危患者诊断价值分析

摘　　要：目的探讨液基薄层细胞学检查(TCT)联合宫颈组织活检对宫颈癌(CC)高危患者的诊断价值。方法选取78例CC高危患者,均接受TCT、宫颈组织活检检查。以术后病理学切片检查结果为金标准,比较两种检查方式阳性检出率,并绘制工作特征(ROC)曲线,运用曲线下面积(AUC)分析TCT、宫颈组织活检两种检查方式诊断价值。结果78例CC高危患者经术后病理学切片检查显示,有33例(42.31%)确诊为CC;运用TCT检查,有36例(46.15%)为阳性,42例(53.85%)为阴性;运用宫颈组织活检检查,有32例(41.03%)为阳性,46例(58.97%)为阴性。两者联合检查AUC显著大于两者单独检查(P<0.05),且联合检查、宫颈组织活检检查诊断敏感度与特异度高于TCT检查(P<0.05)。结论TCT联合宫颈组织活检检查在CC高危患者疾病诊断方面具有较高价值,有利于提高诊断敏感度与特异度,具有较为理想的诊断价值。Objective To explore the diagnostic value of liquid-based thin layer cytology(TCT)combined with cervical tissue biopsy in high-risk patients with cervical cancer(CC).Methods 78 patients with high risk of cC were selected as the study objects,all of whom were examined by TCT and cervical biopsy.With the results of postoperative pathological section as the gold standard,the positive detection rate of the two methods was compared,and the operating characteristic(RoC)curve was drawn,and the diagnostic value of TCT and cervical tissue biopsy was analyzed by using the area under the curve(AUC).Results Of 78 high-risk patients with CC,33 cases(42.31%)were diagnosed with CC.By TCT examination,36 cases(46.15%)were positive and 42 cases(53.85%)were negative.By cervical biopsy,32 cases(41.03%)were positive and 46 cases(58.97%)were negative.The AUC of combined examination was significantly higher than that of single examination(P<0.05),and the diagnostic sensitivity and specificity of combined examination and cervical tissue biopsy were higher than that of TCT examination(P<0.05).Conclusion TCT combined with cervical biopsy has a high value in the diagnosis of CC patients,which is beneficial to improve the diagnostic sensitivity and specificity,and has a relatively ideal diagnostic value.
