显微外科手术治疗HIV患者合并脑肿瘤的临床疗效分析

摘　　要：目的探讨显微外科手术治疗艾滋病病毒(HIV)合并脑肿瘤的临床效果。方法选取60例HIV合并脑肿瘤患者,按随机数字表法分为两组,各30例。对照组予以传统开颅手术治疗,观察组予以显微外科手术治疗。比较两组治疗效果。结果观察组总有效率较对照组高,术后美国国立卫生研究院卒中量表(NIHSS)评分[(9.76±1.23)分]低于对照组[(11.31±1.45)分],Barthel指数(BI)评分[(78.79±7.45)分]高于对照组[(70.54±7.23)分],差异有统计学意义(P<0.05);观察组术后肿瘤坏死因子-α(TNF-α)[(5.14±1.05)ng/L]、白介素-6(IL-6)[(24.56±3.25)ng/L]及中枢神经系统特异性蛋白(S100β)[(8.14±1.13)mg/L]水平及并发症发生率[6.67%(2/30)]较对照组低[(7.58±1.12)ng/L、(28.79±3.32)ng/L、(12.34±2.05)mg/L、26.67%(8/30)],差异有统计学意义(P<0.05)。结论显微外科手术可提高HIV合并脑肿瘤患者治疗效果,减轻神经功能损伤,加快血清学指标复常,且并发症少。Objective To explore the clinical effect of microsurgery in the treatment of AIDS virus(HIV)complicated with brain tumors.Methods 60 patients with HIV and brain tumor were randomly divided into two groups with 30 cases each.The control group was treated with traditional craniotomy,and the observation group was treated with microsurgery.Compare the treatment effect of the two groups.Results The total effective rate of the observation group was higher than that of the control group.The score of the National Institutes of Health Stroke Scale(NIHSS)[(9.76±1.23)]was lower than that of the control group[(11.31±1.45)],and the score of Barthel Index(BI)[(78.79±7.45)]was higher than that of the control group[(70.54±7.23)],with statistical difference(P<0.05);Postoperative tumor necrosis factor in observation group-α(TNF-α)[(5.14±1.05)ng/L],interleukin-6(IL-6)[(24.56±3.25)ng/L]and central nervous system specific protein(S100β)[(8.14±1.13)mg/L]level and complication rate[6.67%(2/30)]were lower than those in the control group[(7.58±1.12)ng/L,(28.79±3.32)ng/L,(12.34±2.05)mg/L,26.67%(8/30)],with statistical difference(P<0.05).Conclusion Microsurgery can improve the treatment effect of HIV patients with brain tumors,reduce the neurological damage,speed up the normalization of serological indicators,and have fewer complications.
