膀胱瓣输尿管吻合术治疗输尿管下段低级别尿路上皮癌的效果分析

摘　　要：目的:分析膀胱瓣输尿管吻合术治疗输尿管下段低级别尿路上皮癌的效果。方法:选取2015年12月—2023年12月白银市会宁县人民医院收治的输尿管下段低级别尿路上皮癌患者80例作为研究对象,采用随机数字法分为对照组和研究组,各40例。对照组实施半尿路切除术,研究组实施膀胱瓣输尿管吻合术。比较两组生活质量、术中出血量、住院时间、治疗效果、术后并发症发生情况。结果:治疗前,两组世界卫生组织生存质量测定量表(WHOQOL-BREF)评分比较,差异无统计学意义(P>0.05);治疗后,两组WHOQOL-BREF评分升高,研究组高于对照组,差异有统计学意义(P<0.001)。研究组术中出血量少于对照组,住院时间短于对照组,差异有统计学意义(P<0.001)。研究组治疗总有效率高于对照组,差异有统计学意义(P=0.003)。研究组术后并发症总发生率低于对照组,差异有统计学意义(P=0.003)。结论:膀胱瓣输尿管吻合术治疗输尿管下段低级别尿路上皮癌的效果显著,可提高患者生活质量,减少术中出血量,缩短治疗时间,降低术后并发症发生率。Objective:To analyze the effect of valvuloureterostomy in the treatment of lower ureteral low-grade urothelial carcinoma.Methods:A total of 80 patients with lower ureteral low-grade urothelial carcinoma were selected as the study objects at Huining County People's Hospital of Baiyin City from December 2015 to December 2023,and were divided into control group and study group by random number method,with 40 cases in each group.The control group underwent hemiurotomy and the study group underwent valvuloureterostomy.The quality of life,intraoperative blood loss,hospital stay,therapeutic effect and postoperative complications were compared between the two groups.Results:Before treatment,there was no significant difference in WHOQOL-BREF scores between the two groups(P>0.05).After treatment,WHOQOL-BREF score increased in both groups,and the study group was higher than the control group,the difference was statistically significant(P<0.001).The intraoperative blood loss of the study group was less than that of the control group,and the hospital stay was shorter,the difference was statistically significant(P<0.001).The total effective rate of the study group was higher than that of the control group,the difference was statistically significant(P=0.003).The total incidence of postoperative complications in the study group was lower than that in the control group,the difference was statistically significant(P=0.003).Conclusion:Valvuloureterostomy is effective in the treatment of lower ureteral low-grade urothelial carcinoma,which can improve patients'quality of life,reduce intraoperative blood loss,shorten treatment time and reduce the incidence of postoperative complications.
