摘　　要：目的:通过测定结直肠腺瘤组织中髓过氧化物酶(MPO)的水平,探讨其在结直肠腺瘤发生发展及“腺瘤-癌”转变过程中的作用。方法:收集80例结直肠腺瘤患者的组织学标本,常规HE染色,采用免疫组织化学方法检测MPO表达。根据HE染色结果分为低级别管状腺瘤(A)组、管状绒毛状腺瘤(B)组、高级别上皮内瘤变和/或粘膜内癌(C)组;根据结肠镜检查腺瘤最大直径分为直径<10 mm(D)组、直径≥10mm(E)组。比较不同结直肠腺瘤癌变风险及不同结肠镜镜检腺瘤大小间MPO的表达水平。结果:腺瘤组织MPO阳性表达率为81.2%。A组呈弱阳性表达,阳性率66.0%;B组弱阳性表达率45.5%,中等强度阳性表达率45.5%;C组中等阳性表达率45.5%,强阳性表达率36.4%,各组两两比较,差异均有统计学意义(P<0.05)。E组MPO阳性表达率及表达强度均高于D组(P<0.05)。结论:MPO在结直肠腺瘤组织阳性表达,MPO可作为监测疾病病程和严重程度评估的组织学指标。Objective:Detecting the level of myeloperoxidase(MPO)in colorectal adenoma,in order to investigate its role in the development of colorectal adenoma and the transtion of“adenoma-carcinoma”.Methods:The histological specimens of 80 patients with colorectal adenoma were collected,stained with HE,and the expression of MPO was detected by immunohistochemistry.Histological specimens were divided into low-grade tubular adenoma(group A),tubular villous adenoma(group B),high-grade intraepithelial neoplasia and/or intramucosal carcinoma(group C)according to the results of HE staining.According to the maximum diameter of adenoma examined by colonoscopy,it was divided into diameter<10mm(d)group and diameter≥10mm(E)group.The expression level of MPO between the groups was compared.Results:The positive expression rate of MPO in adenoma was 81.2%.Group A showed weak positive expression,and the positive rate was 66.0%.In group B,the weak positive expression rate was 45.5%,and the medium intensity positive expression rate was 45.5%.In group C,the medium positive expression rate was 45.5%,and the strong positive expression rate was 36.4%.There was significant difference in pairwise comparison among the groups(P<0.05).Compared with group D,the positive expression rate and expression intensity of MPO in Group E were higher(P<0.05).Conclusion:MPO is positive in colorectal adenomas,and MPO can be used as a histological index to monitor the course and severity of the disease.
