摘 要:目的 探讨多参数MRI征象对非钙化型乳腺腺病与非钙化型乳腺癌的诊断价值。方法 选取收治的非钙化型乳腺癌50例、非钙化型乳腺腺病46例,均经临床病理检查明确诊断,全部病例均经乳腺MRI检查和乳腺X线摄影检查,乳腺X线摄影检查全部病灶均无钙化特征,分析非钙化型乳腺癌和非钙化型乳腺腺病进行多参数MRI检查的影像学特征和鉴别诊断价值。结果 与非钙化型乳腺腺病相比,非钙化型乳腺癌的MRI早期强化率、最大上升斜率、峰值信号强度、最大径均明显增高,而ADC值、达峰时间明显下降,差异有统计学意义(P<0.05);非钙化型乳腺腺病和非钙化型乳腺癌的MRI毛刺征、分叶征、分布、早期强化曲线、晚期强化曲线等指标差异有统计学意义(P<0.05);两组的内部强化方式差异无统计学意义(P>0.05)。结论 非钙化型乳腺癌和非钙化型乳腺腺病的MRI表现虽有部分重叠,但可根据平均ADC值、早期强化曲线、晚期强化曲线、病灶分布、形态等影像学特征予以鉴别。Objective To explore the clinical value of multi-parameter MRI signs in non-calcified breast adenosis and noncalcified breast cancer.Methods 50 cases of non-calcified breast cancer and 46 cases of non-calcified breast adenosis were se⁃lected and diagnosed by clinical pathological examination.All cases were examined by breast MRI and mammography,and all the lesions had no calcification characteristics on mammography.The imaging characteristics and differential diagnostic value of multiparameter MRI examination of non-calcified breast cancer and breast adenosis were analyzed.Results Compared with non-calcified breast adenosis,the early enhancement rate,maximum upward slope,peak signal intensity and maximum diam⁃eter of MRI in non-calcified breast cancer were significantly higher,while the ADC value and time to peak were significantly lower,with statistical significance(P<0.05);There were significant differences in MRI burr sign,lobulation sign,distribution,early enhancement curve,late enhancement curve and other indicators between non-calcified breast adenosis and non-calcified breast cancer(P<0.05);There was no statistically significant difference in the internal reinforcement methods between the two groups(P>0.05).Conclusion Although the MRI findings of non-calcified breast cancer and breast adenosis have some over⁃lap,they can be differentiated according to the average ADC value,early enhancement curve,late enhancement curve,focus distribution,morphology and other imaging characteristics.
