胃癌血清学中癌胚抗原(CEA)、糖类抗原199(CA199)、糖类抗原724(CA724)、胃蛋白酶原I(PGI)和胃蛋白酶原Ⅱ(PGⅡ)的表达及意义探讨

摘　　要：目的分析胃癌血清学中癌胚抗原(Carcinoembryonic Antigen,CEA)、糖类抗原199(Carbohydrate Antigen 199,CA199)、糖类抗原724(Carbohydrate Antigen 724,CA724)、胃蛋白酶原I(Pepsinogen I,PGI)和胃蛋白酶原Ⅱ(PepsinogenⅡ,PGⅡ)的表达和意义。方法选取2021年6月—2023年6月邳州市中医院收治的154例胃癌患者设为观察组,选取同期本院收治的154例胃良性病变患者设为对照组。采用化学发光法检测所有患者CEA、CA199、CA724、PGI、PGⅡ指标水平,绘制受试者操作特征(Receiver Operating Characteristic,ROC)曲线,并基于曲线下面积(Area Under Curve,AUC)评估上述指标的诊断效能。结果观察组CEA、CA199、CA724、PGI、PGⅡ水平均高于对照组,差异有统计学意义(P均<0.05)。经ROC曲线分析,上述指标单独检测中,PGⅡ的AUC最大,为0.820(95%CI:0.724~0.912),CA724最小,为0.591(95%CI:0.488~0.713)。5项指标联合检测的AUC为0.883(95%CI:0.804~0.949),大于单独检测。结论CEA、CA199、CA724、PGI、PGⅡ联合检测的诊断价值较高,可以作为临床辅助诊断胃癌的参考指标。Objective To analyze the expression and significance of carcinoembryonic antigen(CEA),carbohydrate antigen 199(CA199),carbohydrate antigen 724(CA724),pepsinogen I(PGI)and pepsinogen Ⅱ(PGⅡ)in gastric cancer serology.Methods A total of 154 patients with gastric cancer admitted to Pizhou Hospital of Traditional Chinese Medicine from June 2021 to June 2023 were selected as the observation group.A total of 154 patients with benign gastric lesions admitted to our hospital at the same time were selected as the control group.The levels of CEA,CA199,CA724,PGI and PGⅡ in all patients were detected by chemiluminescence method.The receiver operating characteristic(ROC)curve was protracted,and the diagnostic efficiency was estimated based on the area under curve(AUC).Results The levels of CEA,CA199,CA724,PGI and PGⅡin the observation group were higher than those in the control group,the differences were statistically significant(all P<0.05).According to ROC curve analysis,among the indexes above,the AUC of PGⅡ of 0.820 was the highest(95%CI:0.724~0.912),while 0.591 of CA724 was the lowest(95%CI:0.488~0.713).AUC in the combined index detection of the 5 indexes of 0.883 was higher than those in the single index detection(95%CI:0.804~0.949).Conclusion The diagnostic value of combined detection of CEA,CA199,CA724,PGI and PGⅡ by chemiluminescence method is significant,it can be used as a reference index for clinically assisted diagnosis of gastric cancer.
