摘 要:目的探讨前庭康复训练对脑卒中患者平衡功能及步行能力的影响。方法选取首次发病脑卒中患者40例,随机分为治疗组和对照组,各20例。对照组采用常规的平衡功能训练,治疗组在此基础上加用前庭康复训练,3次/d,根据患者耐受情况,3~12 min/次,5 d/周,共干预8周。在干预前(T0)和干预8周后(T1)对患者进行Berg平衡量表(BBS)评分、计时起立-行走测验(TUGT)测试和Tinetti平衡与步态量表(POMA)评估。结果干预后2组患者的BBS评分、POMA评分均较干预前显著提高(P<0.05),治疗组BBS得分高于对照组(P<0.05),干预后2组POMA评分比较无统计学差异(P>0.05)。TUGT测试2组用时均较干预前减少,评分均较干预前降低,干预后治疗组TUGT评分低于对照组(P<0.05)。结论前庭康复训练结合常规平衡功能及步行训练,能更有效地改善脑卒中患者平衡功能及步行时的稳定性和安全性,减少跌倒风险。Objective To observe the effect of vestibular rehabilitation on balance function and walking ability in stroke patients.Methods A total of 40 patients with first-onset stroke were randomly divided into treatment group and control group,with 20 cases in each group.The control group was treated by conventional balance function training,and the treatment group was treated by vestibular rehabilitation training for 3 times/d,3-12 min/time,5 days/week,according to the patient’s tolerance,for a total of 8 weeks.Patients were assessed with the Berg balance scale(BBS)score,the timed up and go test(TUGT)test,and the Tinetti performance oriented mobility assessment(POMA)before the intervention(T0)and after 8 weeks of intervention(T1).Results The BBS score and POMA score of the two groups after intervention were significantly higher than those before intervention(P<0.05),and the BBS score of the treatment group was higher than that of the control group(P<0.05),and there was no difference in the POMA score between the two groups after intervention(P>0.05).The TUGT test time of the two groups reduced compared with that before intervention,and the scores of the two groups were lower than that before intervention.After intervention,the TUGT score of the treatment group was lower than that of the control group(P<0.05).Conclusion Vestibular rehabilitation training combined with conventional balance function and walking training has significant effect on improving the balance function,and the stability and safety of stroke patients when walking,and reduce the risk of falls,which is worthy of clinical promotion.
