不同隐形矫治器清洁方法对错(牙合)畸形患者牙周健康的影响

摘　　要：目的:探讨三种隐形矫治器清洁方法对错(牙合)畸形患者牙周健康的影响。方法:选取笔者医院2021年1月-2021年10月收治的错(牙合)畸形患者60例,均采用无托槽隐形矫治,采用随机数字表法分成三组,每组20例。对照组使用软毛牙刷、单束刷和含氟牙膏机械清洗隐形矫治器。试验1组在对照组的基础上使用保丽净清洁片浸泡隐形矫治器,1周2次,每次30 min。试验2组在对照组的基础上使用醋酸氯己定溶液浸泡隐形矫治器,1周2次,每次30 min。比较三组患者治疗前、治疗1个月、3个月、6个月后的菌斑指数(Plaque index,PLI)、牙龈指数(Gingival index,GI)、探诊深度(Probing depth,PD)、龈沟出血指数(Sulcus bleeding index,SBI)以及矫治器菌斑指数。结果:治疗1个月时,试验1组矫治器菌斑指数低于对照组(P<0.05)。治疗3个月时,试验1组牙龈指数、矫治器菌斑指数低于对照组(P<0.05);试验2组矫治器菌斑指数低于对照组。治疗6个月时,试验1组在牙龈指数、龈沟出血指数、矫治器菌斑指数均低于对照组(P<0.05);试验2组牙龈指数、矫治器菌斑指数均低于对照组(P<0.05)。三组不同时间点患者菌斑指数、探诊深度组间比较差异无统计学意义(P>0.05)。重复测量方差分析显示,对照组内治疗前及治疗后1、3、6个月龈沟出血指数、探诊深度、牙龈指数、矫治器菌斑指数比较差异有统计学意义(P<0.05);试验1组内治疗前及治疗后1、3、6个月牙龈指数比较差异有统计学意义(P<0.05);试验2组内治疗前及治疗后1、3、6个月牙龈指数比较差异有统计学意义(P<0.05)。结论:保丽净清洁片和醋酸氯己定溶液均可有效地清除矫治器菌斑,保丽净清洁片在维护牙周健康作用中效果更佳且使用方便,值得在临床中推广使用。Objective To investigate the effect of cleaning methods of invisible appliance on periodontal health of patients with malocclusion deformity.Methods A total of 60 patients with malocclusion deformity who were treated in the author's hospital from January 2021 to October 2021 were selected.All patients were treated with invisible appliance,and they were grouped according to the random number table method,with 20 cases in each group.In the control group,the invisible appliance was mechanically cleaned with a soft bristle toothbrush,a single bundle brush andfluoride toothpaste.On the basis of the control group,the invisible appliance in trial group 1 were soaked with Polidentt cleaning tablets twice a week for 30 minutes each time.On the basis of the control group,the invisible appliance in trial group 2 were soaked with chlorhexidine acetate solution twice a week for 30 minutes each time.The plaque index(PLI),gingival index(GI),probe depth(PD),sulcus bleeding index(SBI)and invisible appliance plaque index were compared among the three groups before treatment,1 month,3 months and 6 months after treatment.Results After 1 month of treatment,the invisible appliance plaque index in trial group 1 was lower than that in control group(P<0.05).After 3 months of treatment,the gingival index and invisible appliance plaque index of trial group 1 were lower than those of control group(P<0.05),and the invisible appliance plaque index of trial group 2 was lower than that of control group(P<0.05).After 6 months of treatment,the gingival index,sulcus bleeding index and invisible appliance plaque index of trial group 1 were lower than those of control group,the gingival index and invisible appliance plaque index of trial group 2 were lower than those of control group(P<0.05),the invisible appliance plaque index of trial group 1 was lower than that of trial group 2(P<0.05).There was no significant difference in plaque index and probing depth among the three groups at different time points(P>0.05).Repeated measurement analysis of va
