基于“肺朝百脉主治节”理论探讨慢性萎缩性胃炎证治

摘　　要：慢性萎缩性胃炎(CAG)以胃腺减少萎缩为主要特征,可归属于中医学“胃脘痛”“胃痞病”“反胃”等范畴,脏腑辨证病位在胃,是目前常见的一类消化系统疾病,影响因素多样。现阶段中西医结合治疗,或直接采用中医治疗已经成为CAG治疗的必然趋势。中医治病以整体观念为指导思想,《黄帝内经》中记载“肺朝百脉、主治节”,在脏腑功能、五行生克、经络循行等方面,肺胃二者联系密切,在《理虚元鉴》中也记载:“肺气一病,百病蜂起”,在重视脾胃气机升降的同时,从肺着手,注重脏腑同调,防治并举。因此基于“肺朝百脉主治节”理论探讨慢性萎缩性胃炎证治,旨在探寻CAG辨治的基本规律,以期为CAG的临床治疗提供思路和方法,更好地指导中医临床实践。Chronic atrophic gastritis(CAC)mainly characterized by reduced atrophy of the gastric glands,can be attributed to the categories of gastric pain,gastric plague regurgitation in Chinese medicine.The disease is located in the stomach,which is a common digestive disease with various influencing factors.At this stage,the combination of Chinese and western medicine,or the direct use of Chinese medicine has become an inevitable trend in the treatment of CAG.Chinese medicine takes the overall concept as the guiding ideology in treating diseases,and it is recorded in the Yellow Emperor’s Classic of Internal Medicine that“the lungs are in the direction of all the veins,and are the master of the joints”,and the lungs and the stomach are closely linked in terms of the functions of the internalorgans,the birth of the five elements,and the circulation of the meridians,and it is also recorded in the Lixu Yuanjian that when the lungs are sick,all diseases will rise up,in the importance of the spleen and stomach qi lifting and lowering at the same time,from the lungs,focusing on viscera and bowels of the same adjustment,prevention and treatment.Therefore,the discussion on the evidence and treatment of chronic atrophic gastritis based on the theory of the lungs face the hundred veins and the main treatment section aims to explore the basic rules of CAG treatment,with a view to providing a way of thinking and methodology for the clinical treatment of CAG.
