名中医胡世平运用“培养、招纳、解散”三法治疗慢性肝病腹水经验

摘　　要：腹水(单腹胀)是以单纯腹部胀大,青筋暴露,色素沉着为主要临床表现的疾病。阐明治疗单腹胀“培养、招纳、解散”三法源头及具体含义:“培养、招纳、解散”三法为清代医家喻昌治疗单腹胀时率先提出来的有效治法。“培养”即补益元气,“招纳”即升阳运脾,“解散”即疏表利小便。分析单腹胀的病因病机:其一为妄用泻脾之药,其二为病位不明,忽略脾在单腹胀治疗中的核心地位,其三为病治不宜,最终导致脾气衰微,四运不继,气血水不运胶结壅滞腹部。导师胡世平教授在继承先贤理论基础上,进一步提炼丰富“培养、招纳、解散”三法为:“培养”即腹水治脾,大补中气,“招纳”即升阳运枢,妙用风药,“解散”即蠲利水道,气血并疏。Monoabdominal distension is a disease with simple abdominal distension,vein exposure and pigmentation as the main clinical manifestations.To clarify the origin and specific meaning of the three methods of“cultivation,invocation and dissolution”in the treatment of monoabdominal distension:“Cultivation,Invocation and Dissolution”are the effective treatments first proposed by Yu Chang,a doctor in the Qing dynasty,for the treatment of monoabdominal distension.“Cultivation”is to replenish the vital energy,“invocation”is to elevate the yang and transport the spleen,and“dissolution”is to loosen the surface and facilitate urination.Analysis of the aetiology of monoabdominal distension:firstly,the use of spleen-dissolving drugs;secondly,the unknown location of the disease,ignoring the central position of the spleen in the treatment of monoabdominal distention;and thirdly,the inappropriate treatment of the disease,which ultimately led to the weakening of spleen qi and the lack of succession of the four movements of qi and blood,and the lack of transportation of qi,blood and water to the abdomen,which was congested.Professor Hu Shiping in the inheritance of sage theory,further refinement and enrichment of the“cultivation,recruitment,dissolution”of the three methods:“Cultivation”that is,ascites treatment of the spleen,replenishing the middle qi,“Recruitment”that is,to raise the yang transportation centre,the wonderful use of wind medicine,“Dissolution”that is,remit the water course,qi and blood and sparing.
