摘　　要：目的:探究内界膜剥除术联合玻璃体切割术治疗增生性糖尿病视网膜病变伴黄斑脱离的效果。方法:选取2022年10月—2023年10月烟台爱尔眼科医院眼科收治的增生性糖尿病视网膜病变伴黄斑脱离患者80例为研究对象,依据电脑随机数表法分为参照组和研究组,各40例。参照组进行玻璃体切割术,研究组在参照组基础上进行内界膜剥除术。比较两组最佳矫正视力(BCVA)、黄斑中心凹视网膜厚度(CMT)、黄斑中心凹下视网膜下液高度(SFH)、眼压(IOP)及术后并发症。结果:术后3个月,两组BCVA、CMT、SFH水平降低,IOP水平升高,研究组BCVA、CMT、SFH、IOP水平低于参照组,差异有统计学意义(P<0.05)。两组术后并发症发生率比较,差异无统计学意义(P>0.05)。结论:内界膜剥除术联合玻璃体切割术治疗增生性糖尿病视网膜病变伴黄斑脱离效果较好,可改善术后视力,减轻黄斑水肿,降低IOP,安全性好。Objective:To investigate the effect of internal boundary membrane stripping combined with vitrectomy in the treatment of proliferative diabetic retinopathy with macular detachment.Methods:A total of 80 patients with hyperplastic diabetic retinopathy with macular detachment admitted to the Department of Ophthalmology,Yantai Aier Eye Hospital from October 2022 to October 2023 were selected as the study objects,and were divided into the reference group and the study group according to computer random number table method,with 40 cases in each group.The control group underwent vitrectomy,and the study group underwent internal boundary membrane stripping on the basis of the control group.The best corrected visual acuity(BCVA),macular foveal retinal thickness(CMT),subfoveal fluid height(SFH),intraocular pressure(IOP)and postoperative complications were compared between the two groups.Results:3 months after operation,BCVA,CMT and SFH levels decreased and IOP levels increased in both groups,BCVA,CMT,SFH and IOP levels in the study group were lower than those in the reference group,the difference was statistically significant(P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion:Internal boundary membrane stripping combined with vitrectomy is effective in the treatment of proliferative diabetic retinopathy with macular detachment,which can improve postoperative visual acuity,alleviate macular edema,reduce IOP,and has good safety.
