血清ANXA2、CK18、let-7a水平联合检测在喉癌诊断中的效能

摘　　要：目的:分析血清膜联蛋白A2(ANXA2)、细胞角蛋白18(CK18)、let-7a水平联合检测在喉癌诊断中的效能。方法:回顾性分析2021年10月至2023年3月于该院就诊的160例喉部病变患者的临床资料,根据病理学检查结果分为恶性组(n=80)和良性组(n=80)。另选取同期本院80名健康体检者的临床资料,作为对照组。比较三组血清ANXA2、CK18、let-7a水平,比较不同临床分期与分化程度喉癌患者血清ANXA2、CK18、let-7a水平,并绘制受试者工作特征(ROC)曲线,分析血清ANXA2、CK18、let-7a水平单项及联合检测在喉癌诊断中的效能。结果:恶性组血清ANXA2、CK18水平均高于良性组和对照组,且良性组高于对照组;恶性组血清let-7a水平均低于良性组和对照组,且良性组低于对照组,差异有统计学意义(P<0.05);临床分期为Ⅲ期喉癌患者血清ANXA2、CK18水平均高于Ⅰ、Ⅱ期患者,且Ⅱ期患者高于Ⅰ期患者;临床分期为Ⅲ期喉癌患者血清let-7a水平低于Ⅰ、Ⅱ期患者,且Ⅱ期患者低于Ⅰ期患者,差异均有统计学意义(P<0.05);分化程度为低分化喉癌患者血清ANXA2、CK18水平均高于高分化、中分化患者,且中分化患者高于高分化患者;分化程度为低分化喉癌患者血清let-7a水平低于高分化、中分化患者,且中分化患者低于高分化患者,差异均有统计学意义(P<0.05);ROC曲线分析结果显示,血清ANXA2、CK18、let-7a水平单项及联合检测诊断喉癌的曲线下面积分别为0.854、0.837、0.762、0.935,其中联合检测诊断喉癌的效能高于三者单项检测。结论:血清ANXA2、CK18、let-7a水平联合检测诊断喉癌的效能高于三者单项检测。Objective:To analyze efficiency of combined detection of serum annexin A2(ANXA2),cytokeratin 18(CK18)and let-7a levels in diagnosis of laryngeal cancer.Methods:The clinical data of 160 patients with laryngeal lesions who were treated in this hospital from October 2021 to March 2023 were retrospectively analyzed.According to the results of pathological examination,they were divided into malignant group(n=80)and benign group(n=80).In addition,the clinical data of 80 healthy subjects in this hospital during the same period were also retrospectively analyzed and they were selected as the control group.The levels of serum ANXA2,CK18 and let-7a were compared among the three groups.The levels of serum ANXA2,CK18 and let-7a in the patients with different clinical stages and differentiation degrees of laryngeal cancer were compared.The receiver operating characteristic(ROC)curve was drawn to analyze the efficiencies of single and combined detection of serum ANXA2,CK18 and let-7a levels in the diagnosis of laryngeal cancer.Results:The levels of serum ANXA2 and CK18 in the malignant group were higher than those in the benign group and the control group,and those in the benign group were higher than those in the control group;the serum let-7a level in the malignant group was lower than that in the benign group and the control group,and that the benign group was lower than that in the control group;and the differences were statistically significant(P<0.05).The levels of serum ANXA2 and CK18 in the patients with stageⅢlaryngeal cancer were higher than those in the patients with stageⅠand II,and those in the patients with stageⅡwere higher than those in the patients with stageⅠ;the serum let-7a level in the patients with stage III laryngeal cancer was lower than that in the patients with stageⅠandⅡ,and that in the patients with stageⅡwas lower than that in the patients with stageⅠ;and the differences were statistically significant(P<0.05).The levels of serum ANXA2 and CK18 in the patients with low differentiated
