PET/CT评估IA期肺腺癌浸润程度及增殖活性的临床研究

摘　　要：目的探讨正电子发射计算机断层(PET/CT)显像半定量参数对IA期肺腺癌的诊断价值及与肿瘤增殖细胞核抗原(Ki-67)的相关性。方法回顾性分析2021年10月至2023年6月成都市第二人民医院收治的51例IA期肺腺癌患者的PET/CT影像学及病理学资料,按浸润程度不同分为非浸润腺癌组22例和浸润性腺癌组29例,比较两组患者的病灶最大标准化摄取值(SUVmax)及Ki-67表达值的差异,绘制受试者工作曲线(ROC)分析SUVmax对浸润性腺癌的诊断效能,采用Pearson分析SUVmax与Ki-67表达值的相关性。结果浸润性腺癌组患者的平均SUVmax为6.65±1.17,明显大于非浸润腺癌组的1.01±0.53,差异有统计学意义(P<0.05);经ROC分析结果显示,SUVmax=1.75为最佳诊断临界值,敏感度为82.8%,特异度为95.5%;浸润性腺癌组患者的平均Ki-67阳性表达值为(23.34±3.7)%,明显高于非浸润腺癌组的(1.36±0.65)%,差异有统计学意义(P<0.05);经Pearson相关性分析结果显示,Ki-67阳性表达值与SUVmax呈正相关(r=0.725,P<0.05)。结论不同浸润程度的IA期肺腺癌PET/CT表现存在差异,SUVmax对诊断浸润性腺癌有一定帮助,并且与Ki-67表达值显著相关。Objective To investigate the diagnostic value of positron emission tomography/computed tomography(PET/CT)semi-quantitative parameters in stage IA lung adenocarcinoma and its correlation with tumor proliferating cell nuclear antigen(Ki-67).Methods PET/CT imaging and pathological data of 51 patients with stage IA lung adenocarcinoma treated in Chengdu Second People's Hospital from October 2021 to June 2023 were retrospectively analyzed.According to the degree of invasion,they were divided into non-invasive adenocarcinoma group(22 cases)and invasive adenocarcinoma group(29 cases).The maximum standardized uptake value(SUVmax)and Ki-67 expression were compared between the two groups,and the receiver operating curve(ROC)was drawn to analyze the diagnostic efficacy of SUVmax in invasive adenocarcinoma.Pearson analysis was used to analyze the correlation between SUVmax and Ki-67 expression value.Results The average SUVmax in invasive adenocarcinoma group was 6.65±1.17,which was significantly higher than 1.01±0.53 in non-invasive adenocarcinoma group(P<0.05).ROC analysis showed that SUVmax of 1.75 was the best diagnostic critical value,with the sensitivity of 82.8%and the specificity of 95.5%.The average expression of Ki-67 in the invasive adenocarcinoma group was(23.34±3.7)%,which was significantly higher than(1.36±0.65)%in the non-invasive adenocarcinoma group(P<0.05).According to Pearson correlation analysis,the positive expression value of Ki-67 is positively correlated with SUVmax(r=0.725,P<0.05).Conclusion PET/CT findings of stage IA lung adenocarcinoma with different degrees of invasion are different.SUVmax is helpful for the diagnosis of invasive adenocarcinoma and is significantly correlated with Ki-67 expression.
