摘　　要：目的初步探讨对氧磷酶2(PON2)在肝细胞癌(HCC)中的表达水平和临床意义。方法通过分析TCGA数据库和Cbioportal数据库的数据,评估PON2 mRNA的表达水平与HCC临床病理特征和预后之间的相关性并构建预后列线图。为验证PON2在蛋白水平的表达情况,对89份人体HCC组织样本进行免疫组织化学染色,并初步分析PON2蛋白的表达水平与临床病理特征之间的相关性。结果基于TCGA数据库和Cbioportal数据库的数据,发现PON2 mRNA的表达水平与HCC患者的年龄(P=0.038)、肿瘤分级(P=0.005)、甲胎蛋白表达水平(P=0.003)、邻近肝组织炎症(P=0.001)和总生存期事件(P=0.027)显著相关。Kaplan-Meier分析结果显示,PON2 mRNA高表达组的总生存期显著短于低表达组(P=0.004)。预后列线图在预测HCC患者的1、3、5年总生存率方面展现出较高的准确性,一致性指数为0.723(95%CI:0.708~0.737,P<0.05)。免疫组织化学实验结果显示,PON2蛋白在HCC组织中的高表达比例为65.2%,且PON2蛋白高表达与肿瘤分期(P<0.001)和肿瘤分级(P<0.001)呈显著正相关,与血管侵袭(P=0.035)、总生存期事件(P=0.019)和肿瘤复发(P=0.002)显著相关。结论PON2在HCC中的表达水平显著升高,且与诊断和不良预后密切相关。包含PON2 mRNA与多个关键临床因素在内的预后列线图模型精准度较高,能够为临床决策提供有价值的指导性意见。Objective To investigate the expression level and clinical significance of paraoxonase 2(PON2)in hepatocellular carcinoma(HCC).Methods Through analyzing data from TCGA database and cBioPortal database,evaluating the correlation between PON2 mRNA expression level and clinicopathological characteristics as well as prognosis,a nomogram was developed that integrated critical variables.To validate PON2 expression at protein level,immunohistochemical staining was performed in 89 human HCC tissue samples and its correlation with clinicopathological characteristics was analyzed briefly.Results The expression level of PON2 mRNA was correlated with age(P=0.038),tumor grade(P=0.005),AFP level(P=0.003),adjacent hepatic tissue inflammation(P=0.001)and overall survival event(P=0.027)in HCC patients.Kaplan-Meier analysis showed that high expression of PON2 mRNA predicted shorter overall survival than low expression group(P=0.004).Nomogram demonstrated high accuracy C-index of 0.723(95%CI:0.708-0.737,P<0.05)in predicting the 1-,3-and 5-year overall survival of HCC patients.Immunohistochemistry further showed that PON2 protein expression was 65.2%in high expression HCC tissues.High PON2 protein expression was significantly positively associated with tumor stage(P<0.001)and tumor grade(P<0.001)in HCC patients,as well as significantly related to vascular invasion(P=0.035),overall survival event(P=0.019)and recurrence(P=0.002).Conclusions PON2 expression is significantly elevated in HCC and correlated with diagnosis and poor prognosis.The nomogram model,including PON2 mRNA and several key factors,is of high precision and provides valuable guidance for clinical decision-making.
