摘 要:目的:评价医护协同下聚多卡醇液体注射法与泡沫注射法在头颈部静脉畸形中的治疗效果和注射次数的差异。方法:选择采用聚多卡醇硬化剂治疗的头颈部静脉畸形患者120例,其中60例采用液体注射法,60例采用泡沫注射法。根据静脉畸形分型每组分为A、B、C、D 4组,A组为Ⅰ型静脉畸形,B组为Ⅱ型静脉畸形,C组为Ⅲ型静脉畸形,D组为Ⅳ型静脉畸形,共8个亚组,每组15例。比较各组患者的治疗效果与聚多卡醇的注射次数。采用SPSS 23.0软件包对数据进行统计学分析。结果:A组和B组2种注射法比较,治疗效果无显著差异,平均注射次数A组泡沫法减少0.5次,B组减少1.2次。C组和D组2种注射法比较,泡沫注射法治疗效果优于液体注射法,但差异无统计学意义(P>0.05),平均注射次数C组泡沫法减少2.7次,D组减少3.9次。结论:在头颈部静脉畸形治疗中,泡沫注射法对于高回流静脉畸形的疗效优于液体注射法;回流速度越高的静脉畸形,泡沫注射法的平均治疗次数越是少于液体注射法。治疗次数的减少有利于减少患者的经济负担、时间成本以及不良反应的发生率,值得临床推广应用。PURPOSE:To explore the difference in therapeutic effect and injection frequency between polidocanol liquid injection and foam injection in the treatment of head and neck venous malformations under medical and nursing collabo-ration.METHODS:A total of 120 patients with head and neck venous malformations were treated with polidocanol scle-rotherapy,of which 60 were treated with liquid injection and 60 with foam injection.All patients were classified into 4 groups respectively according to the types of venous malformations,namely,group A for type I venous malformation,group B for type II venous malformation,group C for type II venous malformation,and group D for type IV venous malforma-tion.There were a total of 8 groups with 15 cases in each group.The therapeutic effect and the number of polidocanol in-jection were compared between each group.SPSS 23.0 software package was used for statistical analysis of the data.RESULTS:There was no significant difference in therapeutic effect between two injection methods in group A and group B,and the average number of injections was reduced by 0.5 with foam compared to liquid in group A and 1.2 in group B.The therapeutic effect of foam injection was better than that of liquid injection in group C and group D,but the dfference was not statistically significant(P>0.05),and the average number of injections was reduced by 2.7 with foam compared to liquid method in group C and 3.9 in group D.CONCLUSIONS:In the treatment of head and neck venous malformations,the therapeutic effect of foam injection is better than liquid injection for venous malformations with fast drainage.The faster the drainage of venous malformations,the greater is the reduction in average number of treatments with foam injec-tion in relation to that with liquid injection.The reduction in the number of treatments is conducive to reducing patients"economic burden,time consumption and incidence of adverse reactions,and thus foam injection is worth promoting and applying in clinical practice.
