颈段脊旁肌肌电图运动单位电位在诊断神经根型颈椎病中的应用

摘　　要：目的探讨颈段脊旁肌肌电图运动单位电位(MUP)在诊断神经根型颈椎病中的应用价值。方法收集2018年6月至2021年5月就诊于三明市第二医院神经根型颈椎病患者76例,纳入观察组,其中男40例,女36例,年龄40~76岁,病程在12个月以内。另收集82例健康志愿者纳入对照组,其中男42例,女40例,年龄40~76岁。分别对两组研究对象进行正中神经和尺神经运动和感觉传导测定及针极肌电图检测,比较两组正中神经和尺神经运动末端潜伏期(DML)、复合肌肉动作电位波幅(CMAP)、运动传导速度(MCV)及感觉神经动作电位(SNAP)波幅和感觉传导速度(SCV),及MUP时限和波幅。两组均以每10岁作为一个年龄段,比较不同年龄段MUP时限和波幅。结果观察组患者C5~6颈脊旁肌肌电图MUP时限为(14.2±2.9)ms,波幅为(724.6±82.9)μV;对照组MUP时限为(9.3±1.2)ms,波幅为(426.2±62.5)μV。两组MUP时限和波幅比较,差异具有统计学意义(P<0.05)。观察组各个年龄段MUP时限及波幅均高于对照组(P<0.05),组内不同年龄段MUP时限和波幅比较,差异无统计学意义(P>0.05)。两组正中神经、尺神经DML、MCV、CMAP波幅,及SCV和SNAP波幅比较,差异均无统计学意义(P>0.05)。结论神经根型颈椎病患者的C_(5~6)颈段脊旁肌MUP时限延长、波幅增宽,肌电图测定有助于神经根型颈椎病的早期诊断。Objective To explore the application of cervical paraspinal muscle electromyography motor unit potential(MUP)in the diagnosis of cervical radiculopathy.Methods A total of 76 patients with cervical radiculopathy admitted to the Second Hospital of Sanming City from June 2018 to May 2021 were enrolled in the observation group,including 40 males and 36 females,aged 40-76 years old,with disease course within 12 months.Another 82 healthy volunteers were included in the control group,including 42 males and 40 females,aged 40 to 76 years old.The movement and sensory conduction of the median nerve and ulnar nerve were measured and the needle electromyography was performed respectively.The distal motor latency(DML),complex muscle action potential(CMAP)amplitude,motor conduction velocity(MCV),sensory nerve action potential(SNAP)amplitude and sensory conduction velocity(SCV)of the median and ulnar nerves,and duration and amplitude of MUP were compared between the two groups.Both groups use every 10 years as an age group to compare the duration and amplitude of MUP in different age groups.Results The duration of MUP of C5~6 cervical paraspinal muscle in the observation group was(14.2±2.9)ms,and the amplitude was(724.6±82.9)μV.In the control group,the MUP duration was(9.3±1.2)ms and the amplitude was(426.2±62.5)μV.The difference of duration and amplitude of MUP between the two groups was statistically significant(P<0.05).The duration and amplitude of MUP in each age group of the observation group were higher than those in the control group(P<0.05),and there was no statistically significant difference among different age groups within the group(P>0.05).There was no statistical significance in DML,MCV,CMAP amplitude of median nerve and ulnar nerve,as well as SCV and SNAP amplitude between the two groups(P>0.05).Conclusion The duration and amplitude of MUP of C_(5-6)cervical paracarinal muscles in patients with cervical radiculopathy are prolonged and widened,and the EMG determination is helpful for the early diagnosis of c
