基于《金匮要略》虚劳病理论探析卒中后疲劳辨治思路

摘　　要：卒中后疲劳属中医学“虚劳”范畴,其病机以虚为本,因虚致瘀,与肝、脾、肾相关,临床辨证可分为气虚血瘀型、脾胃亏虚型、肝郁脾虚型和肾阳虚衰型。治疗上重视补脾、胃、肾,兼以疏肝、化瘀。代表方剂为补阳还五汤、小建中汤、甘麦大枣汤和八味肾气丸。本文基于《金匮要略》虚劳病理论,为临床治疗卒中后疲劳提供新思路。Post-stroke fatigue belongs to the category of“deficiency fatigue”in TCM,with deficiency as the root cause in pathogenesis,resulting in blood stasis due to deficiency,which is related to the liver,spleen and kidney,and is categorized into the type of qi deficiency and blood stasis,the type of spleen-stomach deficiency,the type of liver-spleen deficiency,and the type of kidney yang deficiency.Treatment emphasizes tonifying the spleen,stomach and kidneys,as well as soothing the liver and removing blood stasis.Representative prescriptions include Buyang Huanwu Decoction,Xiaojianzhong Decoction,Ganmai Dazao Decoction and Bawei Shenqi Pills.This article was based on the theory of deficiency fatigue disease of Jin Gui Yao Lue and provided new ideas for clinical treatment of post-stroke fatigue.
