摘  要:目的:通过与平静呼吸下盆腔磁共振增强扫描比较,探究屏气状态下在盆腔磁共振增强中应用的可行性及图像质量。方法:前瞻性连续收集2019年1月至2023年4月于本院行盆腔磁共振增强检查患者,以平静呼吸下扫描40例作为对照组,同期以屏气状态下扫描40例作为实验组,由2名高年资影像医师采用5分评分标准(1分最差,5分最佳)主观评价两组MRI增强图像的整体图像质量、背景抑制、伪影、生殖系统结构(子宫或前列腺精囊)显示情况,以及客观评价盆腔生殖结构的信噪比(SNR)和对比噪声比(CNR),统计分析两组图像质量的差异性。结果:在主观评价上,以屏气状态下扫描的盆腔磁共振增强图像的整体图像质量、背景抑制、伪影、生殖系统结构显示情况均比平静呼吸下扫描图像质量打分要高,伪影在两组间比较具有统计学差异(P<0.05),其他方面比较没有显著差异(P>0.05)。在客观评价上,屏气状态下扫描的盆腔磁共振增强图像生殖结构的SNR和CNR均比平静呼吸下扫描图像要高,两组比较均具有统计学差异(P<0.05)。另外,2名阅片者之间的主观评分及客观测量SNR和CNR值一致性均较好(Kappa=0.8640.893;ICC=0.897、0.903)。结论:与平静呼吸下盆腔磁共振增强扫描相比,屏气状态下扫描更能活得高质量的图像,而且明显减少图像伪影,满足诊断的要求。Objective:To explore the feasibility and image quality of pelvic magnetic resonance enhancement under breath-holding condition by comparing with pelvic magnetic resonance enhancement scanning under calm breathing.Materials and Methods:The patients who underwent pelvic MRI enhancement examination in our hospital from January,2019 to April,2023 were prospectively and continuously collected.Forty patients were scanned under calm breath as the control group,while 40 patients were scanned under breath-holding condition as the experimental group.Two senior radiologists adopted a 5-point scoring standard(1 point is the worst,5 points is the best)Subjectively evaluate the overall image quality,background suppression,artifacts and the display of reproductive system structure(uterus or prostate seminal vesicle)of the two groups of MRI enhanced images,and objectively evaluate the signal-to-noise ratio(SNR)and contrast-tonoise ratio(CNR)of pelvic reproductive structure,and statistically analyze the differences in image quality between the two groups.Results:In the subjectiveevaluation,the overallimage quality,background suppression,artifacts and the diplay of reproductive system structure of the pelvic MRI enhanced images scanned with breath holding were higher than those of the images scanned with quiet breathing,and the artifacts were statistically different between the two groups(P<0.05),but there were no significant differences in other aspects(P>O.05).Objectively speaking,the SNR and CNR of the reproductive structure of the pelvic MR enhanced images scanned under breath-holding state are higher than those scanned under calm breathing,and there are statistical differences between the two groups(P<0.05).In addition,the subjective score and objective measurement SNR and CNR values between the two readers are in good agreement(Kappa=0.8640.893;ICC=0.897,0.903).Conclusion:Compared with contrast-enhanced pelvic MRI scanning under calm breathing,scanning under breath-holding condition can live a higher quality image,and obviou
