摘 要:目的:观察多层螺旋CT(MSCT)及磁共振成像(MRI)对眼眶性炎性假瘤的诊断效能,并分析其影像学特征。方法:以2017年10月—2021年10月期间我院收治的82例眼眶占位性病变患者为研究对象,开展回顾性研究,根据手术病理结果将其分为假瘤组(42例,病理证实为眼眶炎性假瘤)和肿瘤组(40例,病理证实为眼眶淋巴瘤),两组患者均行MSCT、MRI检查,依据病理诊断结果,统计并对比不同影像学手段对眼眶炎性假瘤的诊断效能,并分析两组患者的MSCT、MRI影像学特征间的差异。结果:MRI对炎性假瘤的诊断准确率90.24%(74/82)略高于MSCT87.80%(72/82)、敏感度92.50%(37/40)略高于MSCT90.00%(36/40)、特异度88.10%(37/42)略高于MSCT85.71%(36/42)(P>0.05);炎性假瘤患者病灶MSCT成像多表现为边缘模糊的弥漫性肿块,可见视神经增粗、眼睑组织肿胀;MRI成像多表现为病灶与眼外肌分界不清,窦腔内有软组织影,T1WI、T2WI信号等或稍长,伴不均匀强化,与淋巴瘤患者的MSCT、MRI成像存在明显差异(P<0.05)。结论:MSCT、MRI对眼眶炎性假瘤的诊断准确率、敏感度、特异度均较高,其MSCT、MRI成像与眼眶淋巴瘤存在明显区别,结合以上影像学检查手段可实现对两类患者的有效鉴别,对辅助诊断炎性假瘤并指导临床治疗均有重要意义。Objective:To observe the diagnostic fficacy of multi-slice spiral CT(MSCT)and magnetic resonance imaging(MRI)in orbital inflammatory pseudotumor,and analyze their imaging characteristics.Methods:A retrospective study was conducted on 82 patients with orbital space occupying lesions admitted to our hospital from October 2017 to October 2021.They were divided into pseudotumor group(42 cases,pathologically confirmed as orbital inflammatory pseudotumor)and tumor group(40 cases,pathologically confirmed as orbital lymphoma)according to the surgical and pathological results.MSCT and MRI were performed on both groups,The diagnostic efficacy of different imaging methods for orbital inflammatory pseudotumor was counted and compared,and the differences of MSCT and MRI imaging characteristics between the two groups were analyzed.Results:The diagnostic accuracy of MRI for inflammatory pseudotumor was 90.24%(74/82),slightly higher than that of MSCT 87.80%(72/82),the sensitivity was 92.50%(37/40),slightly higher than that of MSCT 90.00%(36/40),and the specificity was 88.10%(37/42),slightly higher than that of MSCT 85.71%(36/42)(P>0.05);In patients with inflammatory pseudotumor,most of the lesions on MSCT imaging showed diffuse masses with blurry edges,thickened optic nerves and swollen eyelid tissues;On the surface of MRI imaging,the boundary between the lesion and extraocular muscle was unclear,there was soft tissue shadow in the sinus cavity,TIWI,T2WI signal,etc.or slightly longer,with uneven enhancement,which was significantly different from MSCT and MRI imaging of lymphoma patients(P<0.05).Conclusion:The accuracy,sensitivity and specificity of MSCT and MRI in the diagnosis of orbital inflammatory pseudotumor are high.MSCT and MRI imaging are significantly different from orbital lymphoma.Combining the above imaging methods can achieve effective differentiation between the two types of patients,which is of great significance in assisting in the diagnosis of inflammatory pseudotumor and guiding clinical treatment.
