闭孔疝多层螺旋CT表现及临床分析

摘　　要：目的:分析闭孔疝的多层螺旋CT(multi-slice CT,MSCT)表现及临床特点,以提高对该病的认识水平。资料与方法:对3例经手术治疗及1例保守治疗的闭孔疝患者MSCT资料,包括位置、形态、密度及继发征象观察,结合临床资料进行分析。结果:3例发生于右侧闭孔区,1例发生于左侧闭孔区。MSCT表现:疝囊位于右侧耻骨上支闭孔沟内1例;右侧闭孔内肌与闭孔外肌之间2例,左侧闭孔外肌与耻骨肌之间1例,疝内容物均为小肠,呈结节状软组织密度影和囊状低密度影;3例以肠梗阻症状、1例以右下腹疼就诊。结论:闭孔疝临床表现无特异性,但MSCT对闭孔疝具有确诊价值,怀疑闭孔疝的患者,应常规行腹部MSCT检查。Objective:To analyze the manifestations and clinical characteristics of multi slice spiral CT(MSCT)in obturator hernia,in order to improve the level of understanding of the disease.Materials and Methods:MSCT data of 3 patients with obturator hernia who underwent surgical treatment and 1 patient who underwent conservative treatment were analyzed,including location,morphology,density,and secondary signs.Clinical data were combined for analysis.Result:3 cases occurred in the right obturator area,and 1 case occurred in the left obturator area.MSCT findings:1 case of hernia sac located in the obturator sulcus of the right superior pubic branch;There were 2 cases between the right internal obturator muscle and the external obturator muscle,and 1 case between the left external obturator muscle and the pubic muscle.The contents of the hernia were all small intestine,presenting as nodular soft tissue density shadows and cystic low density shadows;Three patients presented with symptoms of intestinal obstruction,and one patient presented with pain in the lower right abdomen.Conclusion:The clinical manifestations of obturator hernia are not specific,but MSCT has diagnostic value for obturator hernia.Patients suspected of obturator hernia should undergo routine abdominal MSCT examination.
