摘 要:缺血性卒中(ischemic stroke,IS)发病机制复杂,其治疗和康复仍面临众多挑战。针刺作为传统中医治疗手段,因治疗IS有效而受到广泛关注,特别是在调控铁死亡方面。铁死亡是一种铁依赖性脂质过氧化细胞死亡方式,与IS后神经功能损伤密切相关。本文综述了IS后铁死亡的主要机制,尤其强调了铁超载、脂质过氧化、线粒体损伤与铁死亡的关联;并进一步探讨了针刺对IS后铁死亡的调控机制,包括针刺在改善线粒体功能、调节铁代谢、减轻氧化应激、抑制炎症反应以及调节多个铁死亡信号通路方面的作用,以期为IS的综合治疗策略提供更多的科学依据和理论支持。Ischemic stroke(IS)presents a complex pathogenesis that poses numerous challenges in its treatment and rehabilitation.Acupuncture,a traditional Chinese medicine treatment,has garnered widespread attention for its effectiveness in treating IS,particularly its regulation of ferroptosis.Ferroptosis is a type of cell death dependent on iron and lipid peroxidation and is closely linked to neurological impairment following IS.This article reviews the primary mechanisms of ferroptosis post-IS,emphasizing the interconnections among iron metabolism,lipid peroxidation,mitochondrial damage,and ferroptosis.It further explores the mechanisms behind the ability of acupuncture to regulate ferroptosis after IS,including its roles in enhancing mitochondrial function,modulating iron metabolism,alleviating oxidative stress,suppressing inflammatory responses,and influencing various ferroptosis signaling pathways.This paper aims to provide a review of the scientific evidence and theoretical support for comprehensive therapeutic strategies for IS.
