急性视网膜坏死综合征更昔洛韦全身用药时机的临床观察

摘　　要：目的观察急性视网膜坏死综合征(ARNs)不同时机全身使用抗病毒药物更昔洛韦治疗后的视力效果和视网膜脱离发生率,比较更昔洛韦全身用药不同时机对ARNs患者远期视力及视网膜脱离并发症的影响。方法回顾性分析山西省眼科医院2018年6月至2023年6月入院诊断ARNs 22例(23只眼)的资料进行分析,根据开始全身使用更昔洛韦的不同时间点,将患者分为A组(早期治疗组,12只眼,发病2周内开始使用更昔洛韦)和B组(未早期治疗组,11只眼,发病2周后开始使用更昔洛韦)。随访时间1~130周。结果随访末期视力(BCVA)A组高于B组,差异有统计学意义(Z=-2.069,P=0.037),随访末期视网膜脱离发生率A组为25%(3/12),低于B组64%(7/11),差异有统计学意义(P=0.036)。结论ARNs患者早期(2周内)全身使用抗病毒药物更昔洛韦较未早期使用(2周后),远期视力明显改善,视网膜脱离风险减小。Objective To observe the visual effects and incidence of retinal detachment after systemic use of antiviral drug Ganciclovir at different times in acute retinal necrosis syndrome(ARNs),and to compare the effects of different systemic use of Ganciclovir on long-term visual acuity and retinal detachment in ARNs patients.Methods A retrospective case study was conducted to analyze the data of 22 cases(23 eyes)of ARNs admitted to Shanxi Eye Hospital from June 2018 to June 2023.According to the different time points when ganciclovir was administered for systemic use,all the patients were divided into group A(early treatment group,12 eyes,receiving ganciclovir within 2 weeks after onset)and group B(non-early treatment group,11 eyes,receiving ganciclovir after 2 weeks of onset).The follow-up time was 1-130 weeks.Results At the end of follow-up,the visual acuity(BCVA)of Group A was higher than that of Group B,with a statistically significant difference(Z=-2.069,P=0.037).The incidence of retinal detachment at the end of follow-up was 25%(3/12)in Group A,which was lower than 64%(7/11)in Group B,and the difference was statistically significant(P=0.036).Conclusion Early(within 2 weeks)systemic use of the antiviral drug Ganciclovir in ARNs patients has significantly improved long-term vision and reduced the risk of retinal detachment compared to no early use(after 2 weeks)
