毫针针刺联合咽反射治疗对脑卒中后顽固性呃逆患者症状评分、生活质量的影响

摘　　要：目的:观察毫针针刺联合咽反射治疗对脑卒中后顽固性呃逆患者症状评分、生活质量的影响。方法:选取赣南医学院第二附属医院2022年8月—2023年8月收治的脑卒中后顽固性呃逆患者70例,通过随机数字表法分为对照组与试验组,各35例。对照组行咽反射治疗,试验组在对照组基础上毫针针刺足三里、内关、中脘、合谷。均治疗4周记录两组治疗前后症状评分、中医症候积分,采用健康调查量表36(SF-36)评价患者生活质量。结果:治疗后,对照组与试验组症状评分、中医症候积分(呃逆、头晕、痞满、食欲不振)均明显降低,试验组均明显低于对照组(P<0.05)。治疗后两组生活质量(生理功能、角色功能、躯体疼痛、总体健康、精力、社会功能、认知功能和心理健康)评分均明显升高,且试验组均显著高于对照组(P<0.05)。结论:毫针针刺联合咽反射治疗可以降低脑卒中后顽固性呃逆患者呃逆程度,改善生活质量。Objective:To observe the effect of technique of filiform needle acupuncture combined with gag reflex therapy on symptoms scores and quality of life in patients with intractable hiccup after stroke.Method:A total of 70 patients with intractable hiccup after stroke admitted to the Second Affiliated Hospital of Gannan Medical College from August 2022 to August 2023 were selected and divided into control group and experimental group by random number table method,with 35 cases in each group.The control group received gag reflex therapy,and the experimental group received technique of filiform needle acupuncture of Zusanli,Neiguan,Zhongwan and Hegu based on the control group.All patients were treated for 4 weeks.The scores of symptoms and TCM symptoms before and after treatment were recorded in the two groups,and the quality of life of the patients was evaluated by short form 36(SF-36).Result:After treatment,the scores of symptoms and TCM symptoms(hiccup,dizziness,distention and fullness and loss of appetite)in control group and experimental group were significantly decreased,and those in experimental group were significantly lower than those in control group(P<0.05).After treatment,the scores of quality of life(physiological function,role function,physical pain,general health,energy,social function,cognitive function and mental health)in both groups were significantly increased,and those in the experimental group were significantly higher than those in the control group(P<0.05).Conclusion:Technique of filiform needle acupuncture combined with gag reflex therapy can reduce the degree of hiccup in patients with intractable hiccup after stroke and improve the quality of life.
