米库氯铵与顺阿曲库铵在儿童斜视矫正手术中的肌松效果比较

摘　　要：目的比较米库氯铵与顺阿曲库铵用于儿童斜视矫正手术的肌松效果。方法80例拟行儿童斜视矫正手术患儿,随机分为米库氯铵组(M组)与顺阿曲库铵组(C组),每组40例。M组在麻醉诱导期应用米库氯铵,C组在麻醉诱导期应用顺阿曲库铵。比较两组患儿喉罩一次性置入成功率、肌松作用时间、肌松恢复时间、拔除喉罩时间、肌松拮抗剂使用情况、恢复室观察时间及围术期不良事件发生情况。结果两组患儿喉罩一次性置入成功率比较,差异无统计学意义(P>0.05)。M组患儿肌松作用时间(14.5±2.8)min、肌松恢复时间(5.6±1.8)min、恢复室观察时间(19.4±3.7)min明显短于C组的(27.8±2.1)、(9.8±2.1)、(26.6±4.2)min,拔除喉罩时间(8.3±2.1)min显著早于C组的(18.2±1.8)min,肌松拮抗剂使用率5.0%显著低于C组的62.5%(P<0.05)。M组有5例患儿出现皮肤潮红,2例置入喉罩时出现呛咳,3例发生喉罩漏气;C组有2例患儿出现皮肤潮红,2例置入喉罩时出现呛咳,2例发生喉罩漏气。两组患儿围术期不良事件发生率比较差异无统计学意义(P>0.05)。结论米库氯铵应用于儿童斜视矫正手术肌松恢复时间短,拔除喉罩时间快,肌松拮抗剂使用少,在外科快速康复方面具有一定优势。Objective To compare the muscle relaxant effect of mivacurium chloride and cis-atracurium in corrective surgery for strabismus in children.Methods 80 children proposed for strabismus corrective surgery were randomly divided into a mivacurium chloride group(group M)and cis-atracurium group(group C),with 40 cases in each group.Group M was treated with mivacurium chloride during the anesthesia induction period,and group C was treated with cis-atracurium during the anesthesia induction period.Both groups were compared in terms of success rate of one-time laryngeal mask placement,duration of muscle relaxation,recovery time of muscle relaxation,time of laryngeal mask removal,use of muscle relaxation antagonists,length of recovery room stay,and occurrence of perioperative adverse events.Results There was no significant difference in the success rate of one-time laryngeal mask placement between the two groups(P>0.05).Group M had duration of muscle relaxation of(14.5±2.8)min,recovery time of muscle relaxation of(5.6±1.8)min,and length of recovery room stay of(19.4±3.7)min,which were obviously shorter than(27.8±2.1),(9.8±2.1)and(26.6±4.2)min in group C;group M had significantly earlier time of laryngeal mask removal of(8.3±2.1)min than(18.2±1.8)min in group C;group M had significantly lower utilization rate of muscle relaxant antagonists than 62.5%in group C(P<0.05).In group M,5 cases had skin flushing,2 cases had choking when placing the laryngeal mask,and 3 cases had laryngeal mask leakage;in group C,2 cases had skin flushing,2 cases had choking when placing the laryngeal mask,and 2 cases had laryngeal mask leakage.There was no statistically significant difference in the incidence of perioperative adverse events between the two groups(P>0.05).Conclusion Mivacurium chloride used in children's strabismus corrective surgery hhas a short recovery time for muscle relaxation,a fast time for laryngeal mask removal,and less use of muscle relaxation antagonists.It has certain advantages in rapid surgical recovery.
