摘 要:目的 探讨血糖和糖化血红蛋白(Hb A1c)水平与糖尿病视网膜病变(DR)患者病变分级及视力损伤的关系。方法 选取江苏省淮安市第一人民医院分院2019年1月至2022年12月确诊为糖尿病的患者100例,其中DR患者60例(DR组)、糖尿病非视网膜病变(NDR)患者40例(NDR组)。根据DR分期标准将DR组分为35例非增殖型DR患者(NPDR组)和25例增殖型DR患者(PDR组)。比较DR组与NDR组患者、NPDR组与PDR组患者血糖和Hb A1c水平,比较不同空腹血糖(FPG)、餐后2 h血糖(2hPG)、Hb A1c水平患者的视力损伤程度,分析FPG、2hPG、Hb A1c与视力损伤及病变分级的相关性。结果 NDR组FPG、2hPG、睡前血糖、Hb A1c水平低于DR组(P<0.05);NPDR组FPG、2hPG、睡前血糖、Hb A1c水平低于PDR组(P<0.05);不同FPG、2hPG、Hb A1c水平患者视力损伤程度比较,差异有统计学意义(P<0.05);FPG、2hPG、Hb A1c与视力、病变分级呈正相关(r>0,P<0.05)。结论 血糖及Hb A1c水平与DR的发生、病变分级、视力损伤具有相关性,随着血糖及Hb A1c水平的升高,DR患者视力损伤程度增加。Objective To explore the relationship between the levels of blood glucose and glycosylated hemoglobin(Hb A1c) and the pathological grading and visual impairment of patients with diabetic retinopathy(DR).Methods A total of 100 patients diagnosed with diabetes in Branch of Huaian First People's Hospital of Jiangsu Province from January 2019 to December 2022 were selected,including 60 DR patients(DR group) and40 no diabetic retinopathy(NDR) patients(NDR group).According to DR staging standard,the DR group was divided into 35 patients with non-proliferative DR(NPDR group) and 25 patients with proliferative DR(PDR group).The levels of blood glucose and Hb A1cwere compared between DR group and NDR group,NPDR group and PDR group,and the the degree of visual impairment among patients with different levels of fasting plasma glucose(FPG),2 h postprandial plasma glucose(2hPG),and Hb A1cwere compared.The correlation of FPG,2hPG,Hb A1cwith visual imoairment and lesion grading was analyzed.Results The levels of FPG,2hPG,bedtime blood glucose,and Hb A1cin the NDR group were lower than those in the DR group(P<0.05).The levels of FPG,2hPG,bedtime blood glucose,and Hb A1cin the NPDR group were lower than those in the PDR group(P<0.05).There were statistically significant differences in the degree of visual impairment among patients with different levels of FPG,2hPG,and Hb A1c(P<0.05).The levels of FPG,2hPG,and Hb A1cwere positively correlated with visual acuity and lesion grading(r>0,P<0.05).Conclusion Levels of blood glucose and Hb A1care correlated with the occurrence,lesion grading,and visual impairment of DR.As blood glucose and Hb A1clevels increase,the degree of visual impairment in DR patients increases.
