双相情感障碍与广泛性焦虑障碍心率变异性的差异研究

摘　　要：目的探究双相情感障碍(BD)与广泛性焦虑障碍(GAD)人群之间的心率变异性(HRV)的差异。方法将57例BD患者(BD组)和60例GAD患者(GAD组)分别开展HRV检测,并对两者的心率变异相关指标24 h全程NN间期的标准差(SDNN-24)、全程NN间期的标准差(SDNN)、全程每5 min NN间期平均值的标准差(SDANN)、全部相邻NN之差的均方根(rMSSD)、相邻NN之差>50 ms的个数占总窦性心搏个数的百分比(PNN50)、总功率(TP)、低频(LF)、高频(HF)、LF与HF之间的比值(LF/HF)结果进行组间统计差异比较。结果GAD组病程短于BD组(P<0.01)。GAD组的时阈指标SDNN-24、SDANN低于BD组(P<0.01)。相关分析结果显示,GAD组病程与TP、LF、HF、SDNN均呈负相关(P<0.05)。结论GAD的自主神经功能失调要比BD患者更加严重;GAD患者的病程会影响其HRV频阈指标。Objective To explore the differences in heart rate variability(HRV)between patients with bipolar disorder(BD)and generalized anxiety disorder(GAD).Methods A total of 57 patients with BD(BD group)and 60 patients with GAD(GAD group)were selected and tested for HRV,and their HRV-related indicators SDNN-24(24 h-standard diviation of NN intervals),SDNN(standard deviation of NN intervals),SDANN(standard deviation of average NN intervals in all 5 min segments of the entire recording),rMSSD(root mean square difference between adjacent R-R intervals),PNN50(percentage of difference between adjacent R-R intervals>50 ms),TP(total power),LF(low-frequency power),HF(high-frequency power),and the ratio between LF and HF(LF/HF)were compared.Results The disease course of GAD group was shorter than that of BD group(P<0.01).The indicators of SDNN-24 and SDANN in GAD group were lower than those in BD group(P<0.01).Correlation analysis showed that the disease course was negatively correlated with TP,LF,HF and SDNN in GAD group(P<0.05).Conclusion The somatoform autonomic dysfunction of GAD patients is more serious than that of BD patients.The disease course of GAD patients can affect their HRV indicators.
