高压氧治疗用于上颌窦外提升植骨术后早期愈合阶段的免疫组化研究

摘　　要：目的探究早期高压氧干预对上颌窦外提升植骨患者成骨相关因子的影响。方法本研究共招募5名患者,进行上颌窦外提升同期异种异源植骨术。术后3例患者分别接受100%浓度的高压氧治疗5周(T1)、6周(T2)和9周(T3),作为实验组,2例患者自然愈合6周(Ctrl)和13个月(TM),作为对照组。环形骨钻制取对应各组的骨组织块,针对骨钙素和血管内皮生长因子进行免疫组化分析。统计方法采用方差分析和Tukey成对比较分析数据。结果与对照组相比,实验组两种成骨代谢相关因子的表达水平均显著升高(P<0.001)。在对照组中,二因子均未发现显著性差异。在实验组中,骨钙素最低水平出现在T2,而T1和T3组无差异(P=0.0900)。血管内皮生长因子峰值出现在T1,与T2和T3无差异(P=0.0739)。实验组中,血管内皮生长因子和骨钙素都呈现先升高后下降的趋势。结论本研究基于小样本量推断早期应用高压氧治疗5~6周对老年患者上颌窦外提升植骨术后成骨效率的提高有一定意义,该疗法显著增加骨钙素和血管内皮生长因子的代谢水平。Objective To analyze the promotion of osteogenic factors secretion utilizing hyperbaric oxygen(HBO)on sinus floor elevation(SFE)patients.Methods A total of 5 patients that need lateral SFE for implants were enrolled in this study.Pure Xenografts(Bio-Oss,Geistlich,Switzerland)were used for all the patients.After lateral SFE surgery,3 patients as the test groups received 100%HBO therapy for 5 weeks(T1),6 weeks(T2),and 9 weeks(T3),respectively.The other 2 patients healed naturally for 6 weeks(ctrl)and 13 months(TM)as the control groups.Biopsies were harvested and Quantitative histomorphometric analysis were performed regarding two key factors(BPG and VEGF)in different groups.Data was analyzed with ANOVA and Tukey pairwise comparison.Results Significant greater expression for both factors was found on test groups compared to control groups(P<0.001).No significance within the control groups was found for both factors.Within test groups,the lowest level of BPG appeared on T2,while T1 and T3 showed no difference(P=0.0900).For VEGF,the peak appeared on T1,no difference with T2 and T3 was found(P=0.0739).Both BPG and VEGF showed quadratic trends,presenting an initial upward trend and eventually a downward trend depending on T1,T2 and T3 groups.Conclusions HBO therapy at early healing stage after lateral SFE with pure Xenograft significantly improved the secretion of VEGF and BPG.Keeping HBO until 5th to 6th week after surgery may maximized its efficiency on early bone healing stage.
