摘　　要：目的:探讨恶性间皮瘤的细胞、组织学特点及免疫组化特点。方法:收集恶性间皮瘤19例,观察形态学特点,总结免疫组化表达并和同期的浆液性癌比较,复习文献。结果:胸腹水细胞显示细胞量明显增多,细胞呈团状、梁状、乳头状/小簇状排列为主,有的可见开窗现象。组织HE图像:细胞呈腺状及乳头状排列,细胞立方形/圆形,大部分细胞界限尚清。细胞质丰富,胞质红染、淡染、淡红染及紫红染,细胞核圆形、卵圆形,可见明显的小核仁,见少量核分裂象。免疫组化显示:calretinin(CR)、WT-1及D2-40常阳性,CK7、CK5/6及Vimentin常阳性,与浆液性癌比较(P<0.05)(除WT-1及CK7无统计学意义)。p16不等程度的阳性,阳性程度低于浆液性癌(P<0.05)。PAX-8可以阳性(1/5),明显低于浆液性癌(P<0.05)。CEA阴性(0/7)。结论:恶性间皮瘤的形态有其特点,结合免疫组化可以诊断。Objective:To investigate the cellular and histological features of malignant mesothelioma,and immunohistochemical characteristics.Methods:19 cases of malignant mesothelioma were collected,the morphological characteristics were observed,the immunohistochemical expression was summarized and compared with the serous cancer group,and the literature was reviewed.Results:the number of cells in pleural and ascitic fluid increased significantly,and the cells were mainly arranged in cluster,beam,papillary/small cluster.Tissue image:the cells were arranged in glandular and papillary shape.Most of the cell boundaries were still clear.Cytoplasm of the cells was rich with red staining,light staining,light red staining and purplish red staining.The nucleus was round/oval,obvious small nucleoli can be seen,and a few mitotic images can be seen.Immunohistochemistry showed that calretinin(CR),WT-1 and D2-40 were often positive.CK7,CK5/6 and vimentin were often positive(P<0.05)(with no statistical significance except for WT-1 and CK7)comparing with serous cancer group.p16 were positive in varying degrees,lower than serous cancer(P<0.05).Pax-8(+,1/5),significantly lower than serous cancer group(P<0.05).CEA(-,0/7).Conclusion:the morphology of malignant mesothelioma has its own characteristics,which can be diagnosed combined with immunohistochemistry.
