知信行模式护理在食管静脉曲张套扎术后患者中的应用分析

摘　　要：目的:探讨知-信-行方式对胃镜引导套扎食管静脉曲张术后患者疼痛和睡眠的作用。方法:选取2023年3月至2024年3月山东第一医科大学第二附属医院收治的胃镜下食管静脉曲张套扎术治疗患者60例作为研究对象,按照随机数字表法随机分为对照组和观察组,每组30例。对照组实施常规护理,观察组实施知信行模式干预,以疼痛评分(VAS)、睡眠质量评分(PSQI)、负面情绪评分(SAS、SDS)、恢复指标、并发症率、护理满意度为指标展开对照研究。结果:VAS评分比较,术后6 h、24 h、48 h、72 h,观察组更低(P<0.05);护理前,PSQI评分比较,差异无统计学意义(P>0.05),护理后,2组评分均降低,且观察组更低,差异有统计学意义(P<0.05);护理前,SAS、SDS评分比较,差异无统计学意义(P>0.05),护理后,2组评分均降低,且观察组显著低于对照组,差异有统计学意义(P<0.05);恢复指标比较,观察组排气时间与住院时间均显著短于对照组,住院费用低于对照组,差异有统计学意义(P<0.05);观察组并发症率低于对照组,差异有统计学意义(P<0.05);观察组护理满意度高于对照组,差异有统计学意义(P<0.05)。结论:知-信-行干预介入胃镜引导套扎食管静脉曲张,能够减轻疼痛、改善睡眠,同时减轻患者的负面情绪、加快恢复、减少并发症,赢得更高的护理满意度,值得临床普及。Objective:To investigate the effects of epistol administration on pain and sleep in patients with esophagoscope-guided variceal ligation.Methods:A total of 60 patients with esophageal varices ligation under gastroscopy(2023.3 to 2024.3)were selected and divided into an observation group and a control group by random number table method.The control group was given routine nursing,and the observation group was given the intervention mode combining knowledge and practice.Pain score(VAS),sleep quality score(PSQI),negative emotion score(SAS,SDS),rehabilitation index,complication rate and nursing satisfaction were used as control study.Results:The VAS scores of the observation group were lower than those of the control group at 6,24,48,and 72 h after the operation(P<0.05).The PSQI score of the two groups was lower than that of the control group before and after care(P>0.05),and the PSQI score of the observation group was lower than that of the control group(P<0.05).The scores of SAS and SDS before and after care were lower than those of the control group,and the observation group was significantly lower than the control group(P<0.05).Compared with the recovery index,the exhaust time and hospitalization time in the observation group were significantly shorter than those in the control group,and the hospitalization cost was significantly lower than that in the control group,P<0.05;The complication rate of the observation group was lower than that of control group(P<0.05).The nursing satisfaction of the observation group was higher than that of the control group(P<0.05).Conclusion:The ligation of esophageal varices guided by gastroscopy with knowledge,belief and action can relieve pain,improve sleep,reduce patients'negative emotions,accelerate recovery,reduce complications,and win patients'high nursing satisfaction,which is worthy of clinical promotion.
