胃癌组织中EB病毒感染与p53蛋白表达及临床病理特征

摘　　要：目的观察原发性胃癌患者中EB病毒感染情况和p53蛋白的表达及其临床病理特征。方法原位杂交法检测胃癌组织中EB病毒编码的小RNA,分析EB病毒相关性胃癌的病理形态及患者年龄、性别、肿瘤部位、分期分型、分化程度、幽门螺杆菌感染、HER2表达、Ki-67表达等临床病理特征。使用免疫组织化学法检测胃癌组织中p53蛋白的表达水平,分析其与EB病毒感染的相关性。结果101例胃癌标本中检测出12例感染EB病毒(11.9%),与EB病毒非相关性胃癌相比,EB病毒相关性胃癌分化程度低,远处转移多见(χ^(2)=3.53,P<0.05),EB病毒相关性胃癌中p53蛋白的表达率显著高于EB病毒非相关性胃癌(91.7%∶56.2%,χ^(2)=10.871,P<0.05)。结论与EB病毒非相关性胃癌相比,EB病毒相关性胃癌具有独特的临床病理特征,同时EB病毒相关性胃癌中p53蛋白过表达可能与EB病毒感染有关。Objective The purpose of this study is to investigate Epstein-Barr virus infection and expression of the p53 protein and their clinicopathological features in gastric cancer.Methods In situ hybridization was used to detect the expression of EBV encodes a small RNA(EBER)in 101 cases of gastric cancer tissues.The expression of p53 protein was detected by immunohistochemistry in all gastric cancer tissues.These markers along with several tumor characteristics,e.g.,lymphovascular invasion and the extent of differentiation,were analyzed for significant associations by multivariate analysis.Results 12 patients were identified as EBV-positive in 101 cases of gastric cancer.The analysis of clinicopathological features found EBVaGC had relations with degree of differentiation and metastasis(χ^(2)=3.53,P<0.05).The expression of p53 protein was higher in EBVaGC(91.7%)that that in EBVnGC(56.2%)(χ^(2)=10.871,P<0.05).Conclusion There seems existing a few special clinicopathological features in EBVaGC,in which the p53 protein overexpression may have relations with Epstein-Barr virus infection.
