HIFU序贯放疗治疗食管癌伴腹膜后淋巴结转移1例报告

摘　　要：高强度聚焦超声(HIFU)通过高强度聚焦的超声波在局部组织聚集成为一个焦点,使交点处组织的温度迅速升高,组织吸收声能并转化成热能后发生凝固性坏死。本文报道1例食管癌伴腹膜后淋巴结转移患者行2次LFP方案(亚叶酸钙+氟尿嘧啶+顺铂)化疗后病情进展,腹膜区包块较前明显增大,鳞状细胞癌抗原持续增高,后给予2次HIFU序贯放疗,PET-CT示腹膜后病灶形态缩小,病灶肿瘤活性明显减低。该治疗模式取得了良好的疗效。High Intensity focused ultrasound(HIFU)gathers into a focal point in local tissues through high-intensity focused ultrasonic waves,which causes the temperature of the tissue at the intersection point to rise rapidly.The tissue absorbs acoustic energy and converts it into thermal energy,then coagulative necrosis occurs.In this paper,a patient with esophageal cancer and retroperitoneal lymph node metastasis was reported.After two times of chemotherapy with LFP(calcium folinate+fluorouracil+cisplatin)regimen,the peritoneal mass was significantly larger than before,and the squamous cell carcinoma antigen continued to increase.The patients was then given HIFU sequential radiotherapy.PET-CT showed that the shape of the retroperitoneal lesions was reduced and the tumor activity was significantly decreased.This kind of treatment mode achieves good therapeutic effect.
