MRI联合MSCT检查对脑血管疾病的检出率及诊断效能

摘　　要：目的分析磁共振成像(MRI)联合多层螺旋CT(MSCT)检查对脑血管疾病的检出率及诊断效能。方法回顾性抽取2022年3月至2023年3月南阳南石医院收治的112例疑似脑血管疾病患者,均进行MRI、MSCT检查,以脑血管造影结果作为金标准,分析MRI、MSCT检查单独诊断及联合诊断对脑血管疾病的检出率及诊断效能。结果112例疑似脑血管疾病患者经脑血管造影确诊阳性81例,阴性31例。MRI、MSCT单独检查诊断脑血管疾病的结果与脑血管造影一致性kappa值为0.845、0.604,MRI联合MSCT检查与脑血管造影一致性kappa值为0.955。MRI联合MSCT检查诊断脑血管疾病患者的灵敏度(98.77%,80/81)、准确度(98.21%,110/112)、阴性预测值(96.77%,30/31)高于二者单一检查,差异有统计学意义(P<0.05)。81例确诊脑血管疾病患者经脑血管造影证实,其中44例为脑梗死,25例为脑出血,12例为颅内动脉瘤。MRI联合MSCT检查对脑梗死、脑出血、脑血管疾病的检出率均高于MRI、MSCT二者单独检查,差异有统计学意义(P<0.05);3种诊断方式对颅内动脉瘤的检出率比较差异未见统计学意义(P>0.05)。结论MRI、MSCT检查对脑血管疾病均具有一定的诊断价值,且MRI联合MSCT检查可以增加脑梗死、脑出血等各种脑血管疾病的检出率,提升诊断效能。Objective To analyze the detection rate and diagnostic efficacy of magnetic resonance imaging(MRI)combined with multi-slice spiral computed tomography(MSCT)for cerebrovascular diseases.Methods A total of 112 patients with suspected cerebrovascular diseases treated in Nanshi Hospital of Nanyang from March 2022 to March 2023 were selected retrospectively.And they all underwent MRI and MSCT examinations.The results of cerebral angiography were considered as the gold standard to analyze the detection rate and diagnostic efficacy of MRI,MSCT,and combination of them in the diagnosis of cerebrovascular diseases.Results Among the 112 suspected cerebrovascular disease patients,81 patients were confirmed positive,and 31 patients were negative according to cerebral angiography.The kappa value of consistency of MRI and MSCT with cerebral angiography in diagnosing cerebrovascular diseases were 0.845 and 0.604,respectively;while the kappa value of consistency between MRI combined with MSCT and cerebral angiography was 0.955.The sensitivity(98.77%,80/81),accuracy(98.21%,110/112),and negative predictive value(96.77%,30/31)of MRI combined with MSCT in the diagnosis of cerebrovascular disease were higher than those of the two single examinations,and the difference was statistically significant(P<0.05).Results of cerebral angiography identified 81 patients with cerebrovascular disease,including 44 cases of cerebral infarction,25 cases of cerebral hemorrhage,and 12 cases of intracranial aneurysm.The detection rates of cerebral infarction,cerebral hemorrhage,and cerebrovascular diseases by MRI combined with MSCT were higher than those by MRI alone and MSCT alone,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the detection rate of intracranial aneurysms among the three diagnostic methods(P>0.05).Conclusions Both MRI and MSCT have certain diagnostic value for cerebrovascular diseases,and the combination of MRI and MSCT can increase the detection rate of various cerebrovascu
