摘 要:目的使用Pentacam测量拟行近视手术的青年患者的角膜形态参数,以期了解不同近视程度状态下的角膜形态。方法采用回顾性病例研究,收集2021年4月-2022年11月于宿迁市第一人民医院拟行近视手术的青年患者448例883眼,使用IOL-master测量眼轴长,Pentacam测量患者的角膜参数。按等效球镜度数将患者分为I、II、III三组,I组为低度近视组(≤-0.50D且>-3D)191只眼(男154眼,女37眼),II组为中度近视组(≤-3D且>-6D)461只眼(男263眼,女198眼),III组为高度近视组(≤-6D)231只眼(男108眼,女123眼)。使用SPSS26.0软件分析比较各组间角膜参数并进行统计学分析。结果①随着屈光度的增高,眼轴、角膜前表面曲率、AL/CR均逐渐增大,低度近视组分别为(24.87±0.78)mm、(42.61±1.52)D、(3.13±0.08),中度近视组分别为(25.51±0.85)mm、(43.01±1.67)D、(3.24±0.07),高度近视组分别为(26.43±0.9)mm、(43.36±1.42)D、(3.39±0.09),组间比较,均有统计学意义(均P<0.05)。角膜后表面曲率、角膜直径随着屈光度的增高逐渐减小,低度近视组分别为(-6.19±0.26)D、(11.89±0.41)mm,中度近视组为(-6.27±0.22)D、(11.75±0.39)mm,高度近视组分别为(-6.31±0.23)D、(11.69±0.38)mm,组间比较,均有统计学意义(均P<0.05)。角膜厚度、前房深度、角膜体积组间比较,均无统计学意义(均P>0.05)。②屈光度与眼轴显著负相关(r=-0.608,P<0.05),屈光度与角膜前表面曲率显著负相关(r=-0.143,P<0.05),屈光度与AL/CR显著负相关(r=-0.817,P<0.05)。眼轴、角膜曲率及AL/CR均与屈光度显著相关,AL/CR相关性最强。结论在近视度数增高变化的过程中,眼轴及角膜曲率表现为复杂的变化形式,屈光度与AL/CR相关度最大,AL/CR了解近视进展更为敏感。Objective The corneal morphological parameters of myopia patients before refractive surgery were analyzed with Pentacam in order to understand the corneal morphology in different myopia states.Methods Retrospective case studies were used.From April 2021 to November 2022,448 young patients who underwent myopia surgery in our hospital were collected.IOL-master was used to measure the axial length(AL)of the patients and Pentacam was used to measure the corneal parameters.The patients were divided into I,II and III groups according to spherical equivalent(SE).There were 191 eyes(154 eyes for men and 37 eyes for women)in low myopia group(≤-0.50D and>-3D),461 eyes(263 eyes for men and 198 eyes for women)in moderate myopia group(≤-3D and>-6D),231 eyes(108 eyes for men and 123 eyes for women)in high myopia group(≤-6D).SPSS26.0 was used to analyze and compare the corneal parameters between the groups.Results①With the increase of SE,AL and mean keratometry of the front surface of cornea(Kmf)and AL/CR all gradually increased.The value of low-grade myopia group was(24.87±0.78)mm,(42.61±1.52)D and(3.13±0.08)respectively,the moderate myopia group was(25.51±0.85)mm,(43.01±1.67)D and(3.24±0.07),the high-myopia group was(26.43±0.9)mm,(43.36±1.42)D and(3.39±0.09).The comparison between groups was statistically significant(all at P<0.05).Mean keratometry of the back surface of the cornea(Kmb)and corneal diameter gradually decreased with the increase of SE,the value of low-grade myopia group was(-6.19±0.26)D and(11.89±0.41)mm respectively,the moderate myopia group was(-6.27±0.22)D and(11.75±0.39)mm,the highmyopia group was(-6.31±0.23)D and(11.69±0.38)mm.The comparison between the groups was statistically significant(all at P<0.05).②SE was significantly negatively correlated with AL(r=-0.608,P<0.05),SE was significantly negatively correlated with Kmf(r=-0.143,P<0.05),SE was significantly negatively correlated with the AL/CR(r=-0.817,P<0.05).AL,Km and AL/CR were significantly correlated with SE.The AL/CR corre
