摘 要:目的采用磁共振波谱(Magnetic Resonance Spectrum,MRS)技术研究酒精依赖患者代谢物变化特点,并探讨其与认知损害之间的关系,以期发现酒精依赖患者认知损害的神经生物学机制。方法使用重复性成套神经心理状态测验(Repeatable Battery for the Assessment of Neuropsychological Status,RBANS)对60例住院酒精依赖患者和40例健康志愿者的认知功能进行评估,采用MRS观测酒精依赖者额叶、杏仁核关键脑区代谢物变化,并分析MRS与RBANS之间的相关性。结果与正常对照组比较,酒精依赖组在左侧额叶、右侧额叶和左侧杏仁核NAA/Cr值均显著降低(P<0.05),左侧额叶、右侧额叶、右侧杏仁核脑区Cho/Cr值显著降低(P<0.05),右侧额叶脑区Glu/Cr值显著降低,而左侧杏仁核区Glu/Cr显著增高(P<0.05)。酒精依赖患者即刻记忆及注意功能分值与左侧额叶NAA/Cr值及右侧杏仁核Cho/Cr值呈正相关(P<0.05);言语功能分值与左侧额叶Cho/Cr值呈正相关(P<0.05);而左侧杏仁核Glu/Cr与即刻记忆分值呈负相关(P<0.05)。结论酒精依赖者双侧额叶及杏仁核代谢产物异常,可能是酒精依赖者认知功能损害的神经生物学基础。Objective Using magnetic resonance spectroscopy(MRS)to study the characteristics of metabolites in alcoholdependent patients,explore the relationship between metabolites and cognitive impairment and to discover the neurobiological mechanism of cognitive impairment in alcohol-dependent patients.Methods Using repeatable battery for the assessment of neuropsychological status(RBANS)to assess cognitive function of 60 hospitalized alcohol-dependent patients and 40 healthy volunteers,and MRS was used to observe metabolite changes in the frontal lobes and amygdala of alcohol-dependent patients,then the correlation between MRS and RBANS was analyzed.Results Compared with control group,the NAA/Cr values of the left frontal lobe,right frontal lobe and left amygdala in alcohol-dependence group decreased significantly(P<0.05),the Cho/Cr values of left frontal lobe,right frontal lobe and right amygdala decreased significantly(P<0.05),and the Glu/Cr value of right frontal lobe brain region decreased significantly,but that of left amygdala region increased significantly,(P<0.05).Correlation analysis between RBANS and MRS in alcohol-dependence group showed that the scores of immediate memory and attention function were positively correlated with the NAA/Cr value of left frontal lobe and the Cho/Cr value of right amygdala(P<0.05).The score of speech function were positively correlated with the Cho/Cr value of left frontal lobe(P<0.05).While the Glu/Cr value of left amygdala was negatively correlated with immediate attention scores(P<0.05).Conclusion Abnormal metabolites of bilateral frontal lobes and amygdala in alcohol-dependent patients may be the neurobiological basis of cognitive impairment in alcohol-dependent patients.
