海马保护对局限期小细胞肺癌预防性脑照射患者认知功能障碍的疗效

摘  要:目的观察海马保护对局限期小细胞肺癌行预防性脑照射(PCI)患者认知功能障碍等相关指标的影响。方法收集临沂市肿瘤医院2017年1月—2021年2月局限期小细胞肺癌患者82例,根据行PCI是否进行海马保护分为观察组和对照组,每组41例。观察组在行PCI时实行海马保护,对照组在此过程中不实行海马保护。评价两组患者放疗前后的神经认知功能、生活自理能力、不良反应及生存情况。结果两组患者放疗前蒙特利尔认知评估量表(Mo-CA)评分比较,P>0.05。放疗后第3、6和12个月,两组患者Mo-CA评分均低于放疗前(P<0.05)且观察组Mo-CA评分均高于对照组(P<0.05)。两组患者放疗后日常生活活动能力量表(ADL)评分均下降,且3个月及6个月时均为轻度功能障碍,但两组患者ADL评分比较,P>0.05。放疗后12个月观察组ADL评分显著高于对照组(P<0.05)。两组患者放疗不良反应、两年脑转移及生存率比较,P>0.05。结论将海马保护技术应用于PCI技术治疗局限期小细胞肺癌,可改善对患者的神经认知功能及生活自理能力,提高患者的生活质量。Objective To observe the effect of hippocampal protection on cognitive dysfunction of localized small cell lung cancer patients undergoing prophylactic cranial irradiation(PCI).Methods A total of 81 patients with localized small cell lung cancer from Linyi Cancer Hospital from January 2017 to February 2021 were collected.The patients were divided into two groups according to whether hippocampal protection was performed during PCI,41 cases in the observation group was with hippocampal protection and 41 cases in the control group was without hippocampal protection.The neurocognitive function,the activities of daily living,adverse reactions and survival rate of the two groups before and after treatment were compared.Results There was no statistically significant difference in MoCA scores before radiotherapy between the two groups(P>0.05).3,6 and 12 months after radiotherapy,the MoCA scores of the two groups were lower than those before treatment,and the observation group was higher than that of the control group(P<0.05).ADL scores of the two groups decreased after radiotherapy and patients were mild dysfunction 3 and 6 months after radiotherapy(P<0.05),but there was no statistically significant difference between the two groups(P>0.05).12 months after radiotherapy,the ADL score of the observation group was significantly higher than that of the control group(P<0.05).There were no significant differences in adverse reaction,2-year brain metastasis rate and survival rate between the two groups(P>0.05).Conclusion The application of hippocampal protection technology in PCI treatment can improve the neurocognitive function,self-care ability and the quality of life of patients with localized small cell lung cancer.
