凝血指标联合肿瘤标志物检测在结肠癌中的临床意义

摘　　要：目的:检测结肠癌患者凝血功能和肿瘤标志物相关指标的表达情况,分析其与肿瘤发展关系。方法:选取2021年1月至2022年6月于包医二附院消化微创中心确诊的结肠癌患者50例作为观察组,对照组选取同期检查结果正常者的血清标本50例。分别检测两组血清中癌胚抗原(CEA)、糖类抗原-199(CA-199)等肿瘤标志物及凝血酶原时间(PT)、活化部分凝血酶原时间(APTT)等凝血功能相关指标,分析组间及组内不同指标表达差异。结果:与对照组比较,观察组APTT和PT明显降低(P<0.05),CEA、CA-199明显升高(P<0.05)。观察组中,不同临床分期(Ⅰ-Ⅱ与Ⅲ-Ⅳ期)比较,Ⅰ-Ⅱ期较Ⅲ-Ⅳ期PT、APTT时间延长(P<0.05),CEA、CA-199数值降低(P<0.05);不同分化程度(高-中分化与低分化)比较,高-中分化较低分化PT、APTT时间延长(P<0.05),CEA、CA-199数值降低(P<0.05)。结论:结肠癌患者中,肿瘤标志物CEA和CA-199及凝血相关指标APTT和PT均异常表达,联合检测可为临床诊断提供依据;同时,联合检测可对结肠肿瘤分期及分化程度进行预测。Objective:To detect the expression of coagulation function and tumor markers in patients with colon cancer,and to analyze its relationship with tumor development.Methods:A total of 50 patients with colon cancer diagnosed in the Digestive Minimally Invasive Center of the Second Affiliated Hospital of Baotou Medical College from January 2021 to June 2022 were selected as the observation group,and 50 serum samples with normal results were selected as the control group.The serum tumor markers such as carcinoembryonic antigen(CEA)and carbohydrate antigen-199(CA-199)and coagulation function related indicators such as prothrombin time(PT)and activated partial prothrombin time(APTT)were detected in the two groups,and the differences in the expression of different indicators between groups and within groups were analyzed.Results:Compared with control group,APTT and PT in observation group were significantly decreased(P<0.05),CEA and CA-199 were significantly increased(P<0.05).In the observation group,compared with different clinical stages(Ⅰ-ⅡandⅢ-Ⅳ),the time of PT and APTT inⅠ-Ⅱwas longer than that inⅢ-Ⅳ(P<0.05),and the values of CEA and CA-199 were lower(P<0.05);compared with different differentiation degrees(high-middle differentiation and low differentiation),the time of PT and APTT in high-middle differentiation was longer than that in low differentiation(P<0.05),and the values of CEA and CA-199 were decreased(P<0.05).Conclusion:In patients with colon cancer,tumor markers CEA and CA-199 and coagulation-related indicators APTT and PT are abnormally expressed,and combined detection can provide a basis for clinical diagnosis.At the same time,combined detection can predict the stage and differentiation of colon tumors.
