血清miR-21、miR-148b、miR-203、miR-370在急性髓系白血病患者中的水平及意义

摘　　要：目的分析微小核糖核酸-21(miR-21)、微小核糖核酸-148b(miR-148b)、微小核糖核酸-203(miR-203)、微小核糖核酸-370(miR-370)在急性髓系白血病患者中的水平及意义。方法选择2020年5月至2022年11月该院收治的急性髓系白血病患者202例作为急性髓系白血病组,选择同期体检健康者242例作为健康对照组。检测并比较两组血清miR-21、miR-148b、miR-203、miR-370水平,比较治疗后不同疗效(完全缓解和非完全缓解)及不同临床特征急性髓系白血病患者血清miR-21、miR-148b、miR-203、miR-370水平,绘制受试者工作特征(ROC)曲线分析血清miR-21、miR-148b、miR-203、miR-370单独及联合检测对急性髓系白血病的诊断价值。结果急性髓系白血病组血清miR-21水平高于健康对照组(P<0.05),血清miR-148b、miR-203、miR-370水平低于健康对照组(P<0.05)。治疗后,202例急性髓系白血病患者非完全缓解117例,完全缓解85例;非完全缓解急性髓系白血病患者血清miR-21水平高于完全缓解患者(P<0.05),血清miR-148b、miR-203、miR-370水平低于完全缓解患者(P<0.05)。不同性别、年龄、骨髓原始细胞比例的急性髓系白血病患者血清miR-21、miR-148b、miR-203、miR-370水平比较,差异均无统计学意义(P>0.05);白细胞计数>10×10^(9)/L的急性髓系白血病患者血清miR-21水平高于白细胞计数≤10×10^(9)/L的患者(P<0.05),血清miR-148b、miR-203、miR-370水平低于白细胞计数≤10×10^(9)/L的患者(P<0.05)。ROC曲线分析结果显示,血清miR-21、miR-148b、miR-203、miR-370联合检测诊断急性髓系白血病的曲线下面积(AUC)为0.810(95%CI:0.770~0.845),高于miR-21、miR-148b、miR-203、miR-370单项检测诊断的AUC[0.797(95%CI:0.757~0.834)、0.803(95%CI:0.763~0.839)、0.775(95%CI:0.733~0.813)、0.735(95%CI:0.691~0.775)],差异均有统计学意义(Z=2.426,P=0.015;Z=2.152,P=0.033;Z=2.780,P=0.010;Z=3.423,P=0.001)。结论急性髓系白血病患者血清miR-21水平升高,miR-148b、Objective To analyze the levels and significance of miR-21,miR-148b,miR-203 and miR-370 in the patients with acute myeloid leukemia.Methods A total of 202 patients with acute myeloid leukemia admitted and treated in this hospital from May 2020 to November 2022 were selected as the acute myeloid leukemia group,and 242 healthy individuals undergoing physical examinations during the same period were selected as the healthy control group.The serum miR-21,miR-148b,miR-203,and miR-370 levels were detected and compared between the two groups,and the serum miR-21,miR-148b,miR-203 and miR-370 levels were compared among the acute myeloid leukemia patients with different therapeutic effects after treatment(complete remission and incomplete remission)and different clinical characteristics.The receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic value of serum miR-21,miR-148b,miR-203 and miR-370 detection alone and combined detection for acute myeloid leukemia.Results The serum miR-21 level in the acute myeloid leukemia group was higher than that in the healthy control group(P<0.05),while the serum miR-148b,miR-203 and miR-370 levels were lower than those in the healthy control group(P<0.05).After treatment,there were 117 cases of incomplete remission and 85 cases of complete remission among 202 cases of acute myeloid leukemia;the serum miR-21 level in the patients with incomplete remission of acute myeloid leukemia was higher than that in the patients with complete remission(P<0.05),and the serum miR-148b,miR-203 and miR-370 levels were lower than those in the patients with complete remission(P<0.05).There was no statistically significant difference in the serum miR-21,miR-148b,miR-203 and miR-370 levels among the acute myeloid leukemia patients with different genders,ages and ratios of bone marrow primitive cells;the serum miR-21 level in the acute myeloid leukemia patients with white blood cell count>10×10^(9)/L was higher than that with white blood cell count≤10×10^(9)/L(P<0.05),while the se
