血清和房水中Klotho、p21水平与急性前葡萄膜炎患者病情严重程度的相关性

摘　　要：目的:探讨急性前葡萄膜炎(AAU)患者血清和房水中Klotho、细胞周期蛋白依赖性激酶抑制剂1(p21)与病情严重程度的关系。方法:纳入2019年3月—2023年8月本院治疗的77例(77眼)AAU患者为研究对象,依据病情严重程度分为重度组32例和轻度组45例。采用酶联免疫吸附法检测血清和房水中Klotho、p21、IL-6、IL-8、TNF-α、VEGF水平;采用Pearson相关性分析AAU患者血清和房水中Klotho、p21与IL-6、IL-8、TNF-α、VEGF的相关性;采用多因素Logistic回归分析筛选AAU患者病情严重程度的影响因素;采用受试者工作特征(ROC)曲线分析血清和房水中Klotho、p21水平对AAU病情严重程度的评估价值。结果:重度组AAU患者血清和房水中Klotho、p21水平低于轻度组,IL-6、IL-8、TNF-α、VEGF水平高于轻度组,差异有统计学意义(P<0.05)。Pearson相关性分析结果显示,AAU患者血清和房水中Klotho、p21与IL-6、IL-8、TNF-α、VEGF均呈负相关(P<0.05)。Klotho、p21是AAU患者病情严重程度的保护因素(P<0.05)。血清Klotho、p21单独及联合评估AAU病情严重程度的AUC分别为0.728、0.760、0.813,房水Klotho、p21单独及联合评估AAU病情严重程度的AUC分别为0.860、0.862、0.941。结论:重度AAU患者血清和房水中Klotho、p21均低表达,二者与病情严重程度关系密切。Objective:To explore the relationship between serum and aqueous humor Klotho,cyclin dependent kinase inhibitor 1(p21),and severity of acute anterior uveitis(AAU)in patients.Methods:From March 2019 to August 2023,77 AAU patients(77 eyes)treated in our hospital were included as the study subjects.They were separated into a severe group of 32 cases and a mild group of 45 cases based on the severity of condition.Enzyme linked immunosorbent assay(ELISA)was applied to detect the levels of Klotho,p21,IL-6,IL-8,TNF-α,and VEGF in serum and aqueous humor.Pearson correlation was applied to analyze the correlation between Klotho,p21,IL-6,IL-8,TNF-α,and VEGF in serum and aqueous humor of AAU patients.Multivariate Logistic regression was applied to analyze and screen the influencing factors of the severity of AAU.Receiver operating characteristic(ROC)curve was applied to analyze the evaluation value of Klotho and p21 levels in serum and aqueous humor for the severity of AAU.Results:The levels of Klotho and p21 in serum and aqueous humor of the severe group were lower than those of the mild group,while the levels of IL-6,IL-8,TNF-α,and VEGF were higher than those of the mild group(P<0.05).Pearson correlation analysis showed that Klotho and p21 in serum and aqueous humor of AAU patients were negatively correlated with IL-6,IL-8,TNF-α,and VEGF(P<0.05).Klotho and p21 were protective factors for the severity of AAU(P<0.05).The AUC for evaluating the severity of AAU using serum Klotho and p21 alone and in combination was 0.728,0.760,and 0.813,respectively,the AUC for evaluating the severity of AAU using aqueous humor Klotho and p21 alone and in combination was 0.860,0.862,and 0.941,respectively.Conclusion:Patients with severe AAU have low expression of Klotho and p21 in serum and aqueous humor,which are closely related to the severity of condition.
