摘 要:目的比较视频脑电图(VEEG)与常规脑电图(REEG)诊断癫痫的早期效能。方法回顾性分析2019年5月至2023年5月长春市中心医院收治的104例拟诊癫痫患者的临床资料,所有患者均进行VEEG与REEG监测,比较VEEG与REEG监测效果、检出情况及分型、定位诊断。以病史、临床表现及其他辅助检查结果为金标准,比较VEEG与REEG诊断癫痫的早期效能。结果VEEG监测到的痫样放电率、临床发作率均高于REEG,差异有统计学意义(P<0.05)。VEEG癫痫、非癫痫发作检出率均高于REEG,暂不诊断癫痫检出率低于REEG,差异有统计学意义(P<0.05)。41例癫痫患者中,强直阵挛性发作8例,肌阵挛发作7例,复杂部分性发作8例,全面性发作5例,单纯部分性发作6例,失神发作7例;睡眠期出现异常放电24例,清醒期出现异常放电7例,睡眠期和清醒期均出现异常放电10例;明确痫样放电部位32例,包括颞区发作8例,额区发作7例,额颞区发作6例,中央区发作4例,枕区发作4例,颞枕后区发作3例。根据病史、临床表现以及其他辅助检查结果诊断,104例拟诊癫痫患者中,确诊癫痫患者58例,非癫痫患者46例,VEEG早期诊断癫痫的准确度、灵敏度、特异度、阳性预测值、阴性预测值均高于REEG,差异有统计学意义(P<0.05)。结论VEEG可提高痫样放电和临床发作检出率,还能用于癫痫分型和痫灶定位,在癫痫早期诊断中具有较高准确度,值得临床应用推广。Objective To compare the early efficiency of video electroencephalogram(VEEG)and routine electroencephalogram(REEG)in the diagnosis of epilepsy.Methods The clinical data of 104 patients with suspected epilepsy admitted to Changchun Central Hospital were retrospectively analyzed from May 2019 to May 2023,all patients were monitored by VEEG and REEG,and the monitoring effects,detection status,classification,and localization diagnosis of VEEG and REEG were compared.Taken medical history,clinical manifestations,and other auxiliary examination results as the gold standard,the early efficiency of VEEG and REEG in the diagnosis of epilepsy was compared.Results The epileptiform discharge rate and clinical seizure rate monitored by VEEG were higher than those by REEG,and the differences were satistically significant(P<0.05).The detection rates of epileptic seizure and non-epileptic seizure by VEEG were higher than those by REEG,while the detection rate of temporarily undiagnosed epilepsy was lower than that by REEG,and the differences were satistically significant(P<0.05).Among the 41 patients with epilepsy,there were 8 cases of tonic-clonic seizures,7 cases of myoclonic seizures,8 cases of complex partial seizures,5 cases of generalized seizures,6 cases of simple partial seizures and 7 cases of absence seizures;there were 24 cases had abnormal discharges during sleep and 7cases had abnormal discharges during wakefulness,and 10 cases had abnormal discharges during both sleep and wakefulness;there were 32 cases epileptiform discharge sites were identified,included 8 cases of temporal seizures,7 cases of frontal seizures,6 cases of frontal-temporal seizures,4 cases of central seizures,4 cases of occipital seizures and 3 cases of seizures in temporo-occipital posterior region.There 58 cases were diagnosed with epilepsy and 46 cases were non-epilepsy among 104 patients with suspected epilepsy according to the medical history,clinical manifestations and other auxiliary examination results.The accuracy,sensitivity,specificity,p
