超高龄胆胰疾病患者应用ERCP诊治的策略综述

摘　　要：随着社会老龄化的日益加重,超高龄的胆胰疾病患者逐渐增多,此类患者多具有基础状况差、合并症多、病情危重等特点,给临床诊治带来巨大挑战。经内镜逆行胰胆管造影术(endoscopic retrograde cholangiopancreatography,ERCP)适合不宜或无法行外科手术的超高龄患者。本文在作者诊治经验和系统文献检索的基础上,重点介绍超高龄胆胰疾病患者应用ERCP时的术前评估、操作要点和策略以及并发症防治。Along with the social aging intensifies,super-aged population with biliary and pancreatic diseases is dramatically increasing,most of whom are characterized by poor general conditions and more complex comorbidities,bringing great challenges to clinical diagnosis and treatment.Endoscopic retrograde cholangiopancreatography(ERCP)is especially suitable for ultra-elderly patients who are not good candidates for surgery.ERCP has great advantages in the management of biliary and pancreatic diseases,such as choledocholithiasis,biliary-pancreatic malignancies.In this review,we take a systematic literature review and focus on addressing the role of ERCP in the treatment of ultra-elderly patients with biliary-pancreatic diseases.
