龈下刮治根面平整术联合Nd:YAG激光辅助治疗中重度慢性牙周炎的疗效

摘　　要：目的探讨龈下刮治根面平整术联合Nd:YAG激光辅助治疗中重度慢性牙周炎的疗效。方法选择2022年10月~2023年10月浙江省温州市中西医结合医院中重度慢性牙周炎患者78例,按随机数字表法分为根面平整组和激光辅助组,每组39例。根面平整组予根面平整术治疗,激光辅助组行根面平整术激光辅助龈下刮治治疗。比较两组全口牙周指数、细菌学指标以及术后不良反应发生率。结果治疗后,两组附着丧失(AL)、探诊深度(PD)、出血指数(BOP)、牙龈指数(GI)、菌斑指数(PI)比较,激光辅助组更低(P<0.05或P<0.01);两组龈下菌斑BANA值、龈沟液液相苯甲酰精氨酸萘酰胺(BANA)值比较,激光辅助组更低(P<0.05或P<0.01);术后两组各不良反应发生率比较,激光辅助组更低(P<0.05)。结论龈下刮治根面平整术联合Nd:YAG激光辅助治疗中重度慢性牙周炎疗效较好,患者全口牙周指数及细菌学指标得到明显改善,术后不良反应发生率均明显降低,临床应用价值明显。Objective To explore the therapeutic effect of subgingival root planing combined with Nd:YAG laser assisted treatment for moderate to severe chronic periodontitis.Method From October 2022 to October 2023,78 patients with moderate to severe chronic periodontitis were selected from Wenzhou Integrated Traditional Chinese and Western Medicine Hospital in Zhejiang Province.They were randomly divided into a root planing group and a laser assisted group,with 39 cases in each group,using a random number table method.The root planing group received root planing treatment,while the laser assisted group received root planing treatment and laser assisted subgingival scraping treatment.Compare the total periodontal index,bacteriological indicators,and incidence of postoperative adverse reactions between two groups.Results After treatment,the attachment loss(AL),probing depth(PD),bleeding index(BOP),gingival index(GI),and plaque index(PI)were compared between the two groups,and the laser assisted group had lower values(P<0.05 or P<0.01);The BANA values of subgingival plaque and gingival crevicular fluid(GCF)in the two groups were compared,and the laser assisted group had lower values(P<0.05 or P<0.01);The incidence of adverse reactions was lower in the laser assisted group compared to the two groups after surgery(P<0.05).Conclusion The combination of subgingival root planing and Nd:YAG laser assisted treatment has a good therapeutic effect on moderate to severe chronic periodontitis.The patient's overall periodontal index and bacteriological indicators have been significantly improved,and the incidence of postoperative adverse reactions has been significantly reduced.The clinical application value is significant.
