超声造影联合年龄在乳腺BI-RADS 4类结节再分类中的应用价值

摘　　要：目的 探讨超声造影联合年龄在乳腺影像学报告及数据系统(BI-RADS)4类结节再分类中的应用价值。方法 收集成都市第三人民医院常规超声分类为BI-RADS 4类,且经过超声造影评分并有病理结果的116个乳腺结节患者的临床资料。以病理诊断为金标准,构建受试者工作特征(ROC)曲线,计算超声造影评分及年龄诊断乳腺良恶性结节的最佳截断值,并分析其诊断价值。利用超声造影评分及年龄的最佳截断值,对116个BI-RADS 4类结节重新进行分类调整。结果 通过绘制ROC曲线得到超声造影评分的最佳截断值为3.5分,年龄的最佳截断值为50.5岁。超声造影评分的ROC曲线下面积为0.887,其诊断乳腺结节良恶性的敏感性及特异性分别为80.8%、96.7%;年龄的ROC曲线下面积为0.793,其诊断乳腺结节良恶性的敏感性及特异性分别为73.1%、85.6%。BI-RADS 4类结节经超声造影评分联合年龄临界值重新调整后,59个结节降为3类,病理结果均为良性;11个结节升为4c类,病理结果均为恶性。结论 超声造影评分联合年龄对BI-RADS 4类结节进行重新分类后可有效提高乳腺结节诊断的准确性,能为临床诊疗提供参考。Objective To explore the application value of contrast-enhanced ultrasound(CEUS)combined with age in the reclassification of Breast Imaging Reporting and Data System(BI-RADS)4 category nodules.Methods The clinical data of 116 breast nodules(of 108 patients)classified as BI-RADS 4 by conventional ultrasound,scored by CEUS and had pathological reports in the Third People's Hospital of Chengdu were collected.Taking the pathology result as the gold standard,ROC curve was constructed to calculate the optimal cut-off values of CEUS score and age in the diagnosis of benign and malignant breast nodules.And their diagnostic value was analyzed.The 116 BI-RADS 4 nodules were reclassified by using the optimal cut-off values of CEUS score and age.Results ROC curve showed that the optimal cut-off value of CEUS score was 3.5 points,and that of age was 50.5 years.The AUC of CEUS score in the diagnosis of benign and malignant breast nodules was 0.887,the sensitivity was 80.8%,and the specificity was 96.7%.The AUC of age in the diagnosis of benign and malignant breast nodules was 0.793,the sensitivity was 73.1%,and the specificity was 85.6%.After the adjustment of classification by CEUS score and age threshold,59 nodules of BI-RADS 4 were downgraded to BI-RADS 3,all of which were benign,and 11 nodules were upgraded to BI-RADS 4c,all of which were malignant.Conclusion CEUS score combined with age can effectively improve the the diagnostic accuracy of breast nodules and provide more information for clinical diagnosis and treatment.
