摘 要:目的 探究细胞外基质金属蛋白酶诱导因子(CD147)、内皮细胞特异性分子-1 (ESM-1)、小而密低密度脂蛋白胆固醇(sdLDL-C)与急性脑梗死(ACI)患者颈动脉斑块类别及狭窄程度的关联性。方法 选择淄博一四八医院2021年3月至2022年3月收治的80例ACI患者,测定患者颈动脉斑块类别及狭窄程度。比较不同颈动脉斑块类别及狭窄程度患者一般资料、血清CD147、ESM-1、sdLDL-C水平;分析血清CD147、ESM-1、sdLDL-C水平与患者颈动脉斑块类别及狭窄程度的关系;探讨患者颈动脉斑块类别及狭窄程度的影响因素。结果 80例ACI患者中存在不稳定斑块49例(61.25%),稳定斑块17例(21.25%),无斑块14例(17.50%);存在重度狭窄7例(8.75%),中度狭窄16例(20.00%),轻度狭窄30例(37.50%),无狭窄27例(33.75%)。不同颈动脉斑块类别及狭窄程度患者血清CD147、ESM-1、sdLDL-C水平比较,差异有统计学意义(P<0.001);Spearman相关性分析可知,血清CD147、ESM-1、sdLDL-C水平与患者颈动脉斑块类别及狭窄程度呈显著正相关,血清CD147、ESM-1、sdLDL-C水平越高患者颈动脉斑块越不稳定,狭窄程度越严重(P<0.001);Logistic回归分析显示,低密度脂蛋白胆固醇、血清CD147、ESM-1、sdLDL-C均为颈动脉斑块类别的影响因素(P<0.05),总胆固醇、低密度脂蛋白胆固醇、血清CD147、ESM-1、sdLDL-C均为颈动脉狭窄程度的影响因素(P<0.05)。结论 血清CD147、ESM-1、sdLDL-C水平与ACI患者颈动脉斑块类别及狭窄程度有关,可用于预测斑块类别及狭窄程度,指导临床治疗。Objective To investigate the correlation between CD 147,endothelial cell specific molecule-1(ESM-1),small dense low-density lipoprotein-cholesterol(sdLDL-C)and carotid plaque properties and stenosis in patients with acute cerebral infarction(ACI).Methods 80 patients with ACI admitted to the Zibo148th hospital from March 2021 to March 2022 were selected to determine properties of carotid plaques and the degree of stenosis.The general data,serum CD 147,ESM-1 and sdLDL-C levels of patients with different carotid plaque properties and stenosis degrees were compared;The relationship between serum CD 147,ESM-1,sdLDL-C levels and carotid plaque properties and stenosis degree was analyzed;To investigate the influencing factors of carotid plaque and stenosis in patients.Results Of the 80 patients with ACI,49(61.25%)had unstable plaques,17(21.25%)had stable plaques,and 14(17.50%)had no plaques;There were 7 cases(8.75%)with severe stenosis,16 cases(20.00%)with moderate stenosis,30 cases(37.50%)with mild stenosis and 27 cases(33.75%)without stenosis.There were significant differences in serum CD 147,ESM-1 and sdLDL-C levels among patients with different carotid plaque properties and stenosis degrees(P<0.001);Spearman correlation analysis showed that the levels of serum CD 147,ESM-1 and sdLDL-C were significantly positively correlated with properties of carotid plaques and the degree of stenosis.The higher the levels of serum CD 147,ESM-1 and sdLDL-C,the more unstable the carotid plaque and the more severe the stenosis(P<0.001);Logistic regression analysis showed that low density lipoprotein cholesterol,serum CD 147,ESM-1 and sdLDL-C were the influencing factors of carotid plaque(P<0.05),and total cholesterol,low density lipoprotein cholesterol,serum CD 147,ESM-1 and sdLDL-C were the influencing factors of carotid stenosis(P<0.05).Conclusion The levels of serum CD 147,ESM-1 and sdLDL-C are related to properties of carotid plaques and the degree of stenosis in ACI patients,which can be used to predict the nature of plaque and
